Abstract: It has been suggested that the excess lung cancer mortality often observed in compensated silicotics may be due to selection for compensation favouring high risk groups such as smokers, or persons already with a tumour. We investigated these hypotheses using data from a cohort of 1165 silicotics compensated in Quebec. There was a pattern of high risk in the first 2 years of follow up, consistent with selection of persons already with a tumour, or some other bias operating for this time period. Also, smoking was more prevalent among compensated silicotics than in the reference population, biasing the risk upwards. However, correcting for both these biases leaves a significantly elevated SMR of 2.53 (95% CI = 1.95–3.20). [ABSTRACT FROM PUBLISHER]
