| Abstract (English): | The article addresses the integration of Integrative and Complementary Health Practices (PICS) into the Brazilian Unified Health System (SUS), highlighting the complexity of their incorporation, especially at the medium and high complexity levels, where they are more commonly practiced than in Primary Health Care. It discusses the epistemological diversity of these practices, which differ from the hegemonic biomedicine, as well as the conceptual, political, and institutional challenges for their implementation, including the need to recognize their specificities without subjecting them to external epistemologies. The text also emphasizes the importance of a critical convergence among different knowledge systems, traditions, and social actors to promote care practices that respect cultural diversity and the right to health as a common good, overcoming the mere rhetoric of comprehensiveness in the SUS. | Abstract (Portuguese): | O artigo aborda a inserção das Práticas Integrativas e Complementares em Saúde (PICS) no Sistema Único de Saúde (SUS) brasileiro, destacando a complexidade de sua incorporação, especialmente nos níveis de média e alta complexidade, onde são mais praticadas do que na Atenção Primária à Saúde. Discute-se a diversidade epistemológica dessas práticas, que diferem da biomedicina hegemônica, e os desafios conceituais, políticos e institucionais para sua implementação, incluindo a necessidade de reconhecer suas especificidades sem submetê-las a epistemologias externas. O texto também enfatiza a importância de uma confluência crítica entre diferentes saberes, tradições e atores sociais para promover práticas de cuidado que respeitem a diversidade cultural e o direito à saúde como bem comum, superando a mera retórica da integralidade no SUS.
