It is about our wish to move into homes in the community after years in hospitals. In it we explain the experiences we had of services in the past and what we are looking forward to for the future. We felt that preparing to move on was very exciting. We also thought about what we would be leaving behind and tried to make sense of it all.</p> <p>Keywords: learning disability; moving; user views; Group therapy</p> <p>Spencer Close is a hospital‐based service for people with learning disability in Epping, Essex. It includes a five‐bedded inpatient unit for people with mental health problems or challenging behaviour, a 10‐bedded long‐term care unit, a day centre and offices. It is staffed by nurses, doctors, psychologists, occupational therapists, speech therapists, a physiotherapist and care workers.</p> <p>Arlene McLean is an Occupational Therapist, and Andre Strydom is a Specialist Registrar in Psychiatry at Spencer Close. Angela Hassiotis is a Consultant Psychiatrist.</p> <p>One of the problems that health staff and patients alike often faced was that patients tended to stay in the inpatient unit for a long time because of lack of suitable accommodation in the community. Patients were frustrated by the difficulties in moving on and this frequently led to confrontation with staff. During the ward rounds, we discussed how we could help our patients to think more about their discharge. Our aims were (<reflink idref="bib1" id="ref1">1</reflink>) to identify concerns about moving, (<reflink idref="bib2" id="ref2">2</reflink>) to share experiences of moving or change, (<reflink idref="bib3" id="ref3">3</reflink>) to work out ways to cope in a new environment. Our expectation also was that it would increase staff awareness about the patient's point of view and create a sense of working together in the inpatient unit. We considered that the best way of doing so would be in a group where those who were about to be discharged were invited to participate. A female patient declined to take part.</p> <p>Arlene and Andre took on the responsibility to run the 'leaving group'. During the life of the group we met regularly to review progress and emerging themes and to set the goals for the next session. Arlene developed the idea of using the house diagrams in order to focus the sessions and provide a visual cue for discussion. The group leaders recorded part of the sessions and contributions 'word for word' by writing them down.</p> <p>As the group continued we became aware of several interesting ideas that the participants brought up about their personal histories and what they wanted of their future homes. We thought that their observations and comments were perceptive and provided us with a different perspective on their views.</p> <p>This, and Malcolm and Kenneth's obvious sense of accomplishment led us to think that turning it into a paper would be a positive outcome for the group and could make a contribution to the issue of ordinary living for people with learning disabilities. It was developed over several months and Kenneth and Malcolm wrote their sections as part of their day activities. We all met to talk about what we wanted to say and how to do that. Malcolm and Kenneth had clear views about what they wanted to include in the paper but Andre, Arlene and Angela helped them to structure their ideas. What follows is in their own words, except for the conclusion.</p> <hd id="AN0009852135-2">Who are we?</hd> <p>First we want to tell you who we are and the services we have been in contact with up to now.</p> <p>My name is Malcolm Garrett and I'm 31 years old. I lived with my mum and dad, two brothers and a sister and went to the Mead School in Harlow until I was 15, then moved to Loughton School until I was 16.</p> <p>I went on to a day care centre and did money skills then to a school where I did cooking, I liked cooking. I then worked at a workshop packing tea bags – 10 in a bag. I liked the job for a while but I got bored. I walked out but went back for my wages and to say sorry. My next job was through the job centre working at the Moat House in Harlow washing up in the kitchen, which I found very boring. I got sent home from this job for chasing the girls with water.</p> <p>Before coming to Spencer Close, I went to a hospital near Royston, then a hospital in Northampton, then Spencer Close.</p> <p>My name is Ken Banham and I have also been admitted to Spencer Close, Epping. My wishes at the moment are to continue to live in Essex because it's a nice area, but in a different place.</p> <p>I lived at home with my mum and dad, two brothers and two sisters. I am the oldest and I lived with them until I was 15 years old. I got into trouble and was sent to a Remand Centre. While I was there, a doctor came to see me and I was sent to hospital in Colchester for about 1 year then to a hostel near Harwich for 1 month. I had problems with the other clients and was then sent back to hospital, but I am not sure how long I lived there before being sent to another hospital for a long time but I'm not sure how long. When I was living at the hospital, I went to a community home in Clacton for a short stay then back to hospital. I returned to the home and lived in a cottage. At the cottage, I had trouble with some children, and was sent back to hospital.</p> <p>My next move was to Spencer Close for 2 months, then a long stay unit in Harlow for another 2 months for a trial period but it didn't work out well so I had to go back to Spencer Close. After this, I went to a big community house for a trial period to see if I liked the place but while I was there, I got into trouble again. Because of this, I came back to Spencer Close where I have lived for about 4 years and am now looking forward to moving on.</p> <hd id="AN0009852135-3">Spencer Close</hd> <p> <emph>Ken</emph>:</p> <p>I have enjoyed some of the stay at Spencer Close and some of the staff were good. Some were not so good. Some of the clients are my friends, but some are not. I have enjoyed the session work that I have done with the day centre staff and O.T. staff.</p> <p> <emph>Malcolm</emph>:</p> <p>I enjoy my sessions here. The bungalow where I live is noisy, but I like my bedroom. I don't have any special friends at Spencer Close and I don't think I have changed since I've been here. There are another three people sharing the bungalow with us; two men and one woman. We both get fed up with the bungalow and the people there sometimes. We get frustrated there. In the time that we have been on the bungalow, staff and clients have come and gone. There will be more changes in the future.</p> <hd id="AN0009852135-4">The leaving group</hd> <p>Because we were leaving, we started the 'leaving group'. The group was run by Arlene and Andre. We had six sessions. Each one lasted about 45 min. We talked about different things. We talked about how to prepare for leaving, what we were worried about, and how to get on with new people. We used a picture of a house to get us started (house picture 1).</p> <p>Graph: Picture 1 The house picture.</p> <p>It was very interesting, and we wrote down some of the things that we said. At the end, we met with Dr Angela Hassiotis to talk about the group, and to decide what to do next. We all agreed to write a paper about it for other people to read. We wrote down our worries about moving on (house picture 2). We made a list about what to do to prepare for a move. We will miss the groups at Spencer Close. Kenneth will miss his job. You may miss some people. Talking about moving on reminded us about other people that had moved on. We wondered how they were getting on. We thought about how to keep contact with the bungalow when we move (house picture 4).</p> <p>Graph: Picture 4 Keeping links.</p> <p>While we were writing the paper, we went to see places where we may move. This is what we thought:</p> <p> <emph>Kenneth went to see a place in Devon</emph> </p> <p>I like the place, the only thing I don't like is they will keep my lighter. The place in Devon has normal wooden chairs, nice furniture and pictures, and the TV is not bolted down. It is like your home. I took photos of the place. I took one of the TV cabinet, I want to show them on the bungalow how different it is. I took pictures of all the care workers. The radio stations in Devon is different. ITV Carlton is also different. I don't know about the funding for the new place in Devon. The panel will also try to look for a place in Essex.</p> <p> <emph>Malcolm had been to Colchester to see a new hospital</emph> </p> <p>I met the staff and a psychologist. I have to see them again. It feels more like a home than Spencer Close.</p> <hd id="AN0009852135-5">Ending the group and moving on</hd> <p>Since we all started working together on the group, things have changed for many of us. Ken has now moved to a residential place in Devon.</p> <p>He has had a farewell party the day before and has called Spencer Close a few times. He has recently written to say that he is doing well and is much happier than at Spencer Close. He has many activities and would like to do more writing. Malcolm is still at Spencer Close waiting until a vacancy is available in Colchester to move into. He said that he is unhappy waiting and upset about the termination of a relationship. He would prefer to talk about his difficulties but he does not plan to do any more writing. Both Kenneth and Malcolm said that the group has helped them with planning and understanding something about their behaviour.</p> <p>Angela and Andre have moved to new jobs in London, and Arlene continues her work at Spencer Close. Working on this paper helped us all to keep contact, and to make sense of change.
