Abstract (English): Basal cell carcinoma (BCC) is the most common skin cancer in humans. Histopathology is the gold standard for its diagnosis. In recent years new non-invasive diagnostic techniques have been developed. Dermoscopy increases diagnostic accuracy in pigmented and no pigmented lesions and helps differentiating BCC from other skin tumors. Reflectance confocal microscopy (RCM) allows the visualization of the superficial layers of the skin with a cellular resolution and could increase dermoscopic diagnosis. Most of BCC characteristics are visualized at superficial dermis or dermoepidermal junction. Several studies have reported criteria for BCC diagnosis. The differences of dermoscopy and RCM criteria between BCC subtypes have been described. These techniques are very useful in patients with history of multiple melanoma, familiar melanoma, dysplastic nevus syndrome and in patients with other genetic syndromes. They help us reduce the number of excised lesions and diagnose skin cancer in early stages. It is important for dermatologists to have knowledge of the basic dermoscopic criteria for the diagnosis of suspicious lesions. In the future with the development of new technology and equipment we will be probably capable to do bed-side diagnosis of skin cancer before surgery and of other skin diseases without performing skin biopsies. Abstract (Spanish): El carcinoma basocelular es la neoplasia cutánea más frecuente en los humanos. El patrón de referencia para su diagnóstico es el estudio histopatológico. En los últimos años se han desarrollado nuevas herramientas no invasivas, auxiliares en el diagnóstico clínico. La dermatoscopia es una técnica que incrementa la precisión diagnóstica en lesiones pigmentadas y no pigmentadas y que facilita la diferenciación de carcinoma basocelular de otros tumores cutáneos. La microscopia de reflectancia confocal (RCM) produce imágenes de la piel en plano horizontal con resolución celular y mejora el diagnóstico dermatoscópico. La mayor parte de las características del carcinoma basocelular se observa en la dermis superficial o en la unión dermoepidérmica y existen criterios bien definidos para establecer su diagnóstico. Actualmente se conocen diferencias significativas en los criterios dermatoscópicos y de microscopia de reflectancia confocal entre los diferentes subtipos de carcinoma basocelular. Estas técnicas son de gran utilidad en pacientes con antecedentes de melanoma múltiple, melanoma familiar, síndrome de nevo displásico y pacientes con otros síndromes genéticos. Ayudan a disminuir el número de lesiones extirpadas y a realizar diagnósticos en estadios tempranos. Es importante que los dermatólogos tengan conocimiento de los criterios básicos de dermatoscopia para el diagnóstico de lesiones sospechosas. En un futuro, gracias al avance tecnológico y al desarrollo de nuevos equipos será posible realizar diagnósticos prequirúrgicos no sólo de cáncer, sino también de otras enfermedades cutáneas sin la necesidad de realizar una biopsia.
