Abstract (English): Drug reaction with eosinophilia and systemic symptoms (DRESS) is a potentially fatal cutaneous reaction to drugs, which has an unclear pathogenesis. It is commonly associated with fever, lymphadenopathy, visceral involvement, and hematologic disorders including eosinophilia. Herein, we present the case of a 22-year-old man who had been receiving antibiotics and analgesics for a respiratory infection, and developed an exanthem that spread throughout most of the body, fever, altered liver, and kidney tests. The skin biopsy was consistent with a drug reaction. The patient notably improved after discontinuation of the offending drugs and corticosteroid treatment. Prompt diagnosis through clinical and laboratory criteria is crucial given the multisystemic and potentially fatal nature of this disease. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): La reacción a medicamentos con eosinofilia y síntomas sistémicos (Dress) es una toxicodermia con alto riesgo de letalidad y cuya patogénesis es poco clara, que se asocia con fiebre, linfadenopatías, afección visceral y alteraciones hematológicas que generalmente incluyen eosinofilia. Se presenta el caso clínico de un hombre de 22 años, quien, tras la ingesta de antibióticos y analgésicos por infección respiratoria, presenta exantema generalizado, fiebre, alteración de pruebas hepáticas y renales. La biopsia de piel confirma reacción medicamentosa y se evidencia mejoría notoria tras la suspensión de los medicamentos sospechosos y la administración de corticoides. Es de vital importancia realizar un diagnóstico temprano mediante criterios clínicos y de laboratorio por parte del personal médico, por el carácter multisistémico y letalidad potencial de esta patología. Abstract (Portuguese): A reação a medicamentos com eosinofilia e sintomas sistémicos (Dress) é uma toxicodermia com alto risco de letalidade e cuja patogénese é pouco clara, que se associa com febre, linfadenopatia, afecção visceral e alterações hematológicas que geralmente incluem eosinofilia. Se apresenta o caso clínico de um homem de 22 anos, quem, após a ingestão de antibióticos e analgésicos por infecção respiratória, apresenta exantema generalizado, febre, alteração de provas hepáticas e renais. A biopsia de pele confirma reação medicamentosa e se evidencia melhoria notória após a suspensão dos medicamentos suspeitosos e a administração de corticoides. É de vital importância realizar um diagnóstico precoce mediante critérios clínicos e de laboratório por parte do pessoal médico, pelo carácter multissistêmico e letalidade potencial desta patologia.
