Abstract (English): Introduction. Having a network of laboratories with capacity to perform microscopic diagnosis that meets WHO/PAHO quality standards and allows the provision of adequate treatment of cases, is required to reach the goal of malaria elimination in Ecuador for the year 2020. Objective. To know the competences of parasitological diagnosis in the microscopists from the local public network and performance in reference laboratories from the intermediate level. Materials and methods. Descriptive cross-sectional study based on information collected in workshops carried out to evaluate the competencies of microscopic diagnosis in the laboratory network from zonal health coordinations 1 to 8, through a slide panel to evaluate the agreement on diagnosis. In addition, diagnostic performance of the intermediate laboratories in an external quality assessment program was evaluated. The results obtained were compared with reference results of the supranational laboratory of Peru. Results. The competencies of 191 microscopists were evaluated In 11 workshops, 153 (80.1%) participants were approved. The medians of the indicators were: concordance for parasite detection 100% (95% CI: 100-100), concordance for species identification 100.0% (95% CI: 100-100), concordances for stage identification 93.0% (95% CI: 91-96) and concordance for parasite counting 77.0% (95% CI: 73-86). In the external quality assessment program, the three intermediate laboratories obtained 100% for the concordance for parasite detection and 96% for concordance species detection. Conclusions. The competence results in primary network and performance indicators in the intermediate laboratories reached high-quality standards related to the training program implemented in the country. Abstract (Spanish): Introducción. Para eliminar la malaria en Ecuador para el 2020 es necesario contar con capacidad para realizar diagnóstico microscópico que cumpla con estándares de calidad de la Organización Mundial de la Salud (OMS) / Organización Panamericana de la Salud (OPS) y permita proveer tratamiento adecuado a los casos. Objetivo. Conocer las competencias para el diagnóstico parasitológico de microscopistas de la red pública local y el desempeño de laboratorios referentes intermedios. Materiales y métodos. Estudio descriptivo de corte transversal a partir de información recogida en talleres que evaluaron las competencias del diagnóstico microscópico en la red de laboratorio de las coordinaciones zonales de salud, a través de un panel de láminas para evaluar la concordancia del diagnóstico. Además, se calificó el desempeño del diagnóstico de los laboratorios intermedios en el Programa de Evaluación Externa de Desempeño (PEED). Los resultados se compararon con los obtenidos por el laboratorio supranacional de Perú. Resultados. En los 11 talleres se evaluaron las competencias de 191 microscopistas, aprobaron 153 (80,1%) participantes. Las medianas de los indicadores fueron: concordancia de detección/resultado 100% (IC 95%: 100-100), concordancia de especie 100% (IC 95%: 100 -100), concordancias de estadio 93,0% (IC 95%: 91-96) y concordancia de recuento 77% (IC 95%: 73-86). En el PEED, los tres laboratorios intermedios obtuvieron concordancia de resultado del 100% y de especie del 96%. Conclusiones. Los indicadores de competencia de la red local y de desempeño en laboratorios intermedios alcanzaron altos estándares de calidad relacionados con el proceso de entrenamiento implementado en el país.
