Abstract (English): The current pandemic caused by the sArs-cov-2, which causes the covId-19 disease, represents an unprecedented challenge for the entire health system, and more especially for the intensive care units (Icu), given that the most cases with severe symptoms can be complicated by acute respiratory distress syndrome (Ards), sepsis / septic shock, and multiple organ failure, including acute kidney injury and damage. Many questions about clinical management remain unanswered, including the importance of myocardial dysfunction and the role of non-invasive ventilation, the high-flow nasal cannula, corticosteroids, and various reused and experimental therapies. the adequacy of life support treatments is a clinical decision used in intensive care units (Icu) to avoid therapeutic obstinacy and futile treatments. since no specialized medications have been identified to treat sArs-cov-2 infection at this time, the mainstay of treatment has been supportive care. the management of the airway, as it is a procedure with a high risk of transmission of the virus, must be carried out under certain conditions that minimize the exposure of personnel without compromising the safety and efficacy required for the procedure. Icu professionals, hospital administrators, governments, and policymakers must prepare early for a substantial increase in critical care capacity, or risk being overwhelmed by the pandemic. Among the main challenges and difficulties faced by medical and nursing staff is identifying the most critical cases of respiratory failure, following intubation and mechanical ventilation protocols, and avoiding contagion among health professionals. Abstract (Spanish): La actual pandemia provocada por el coronavirus sArs-cov-2, causante de la enfermedad covId-19, representa un desafío sin precedentes para todo el sistema sanitario, y más especialmente para las unidades de cuidados intensivos (ucI), dado que los casos más severos pueden complicarse con síndrome de dificultad respiratoria aguda (sdrA), sepsis/shock séptico y fallo multiorgánico, incluyendo daño renal agudo y daño cardíaco. Muchas preguntas sobre el manejo clínico siguen sin respuesta, incluida la importancia de la disfunción miocárdica y el papel de la ventilación no invasiva, la cánula nasal de alto flujo, los corticosteroides y varias terapias reutilizadas y experimentales. dado que en este momento no se ha identificado ningún medicamento especializado para tratar la infección por sArs-cov-2, el pilar del tratamiento ha sido la atención de apoyo. el manejo de la vía aérea por tratarse de un procedimiento de alto riesgo de transmisión del virus, debe realizarse bajo ciertas condiciones que disminuyan al máximo la exposición del personal sin comprometer la seguridad y eficacia que se requiere para el procedimiento. entre los principales retos y dificultades que enfrenta el personal médico y de enfermería está identificar los casos más críticos de insuficiencia respiratoria, seguir protocolos de intubación y ventilación mecánica y evitar contagios entre los profesionales de la salud.
