| Abstract (English): | BACKGROUND: Stevens-Johnson syndrome is a rare and life-threatening mucocutaneous disease. Its main etiology is hypersensitivity secondary to drug intake, where carbamazepine, allopurinol and sulfonamides stand out. Paracetamol as a causal agent has been described in only a few reports and case series, predominantly in Asian population. CLINICAL CASE: A 3-years-old male patient with a history of penicillin allergy. He presented erythematous-violaceous macules on the trunk secondary to the administration of paracetamol to relieve abdominal and testicular pain. He went to the emergency room where he received antihistamines and the dose of paracetamol was increased. After this, he began to exhibit blisters and areas of detached skin with systemic affection. Due to this, he was hospitalized in the pediatric intensive care unit where the diagnosis of Stevens-Johnson syndrome was established. His main treatment was focused on immunoglobulin and steroids, with which he presented progressive clinical improvement. CONCLUSIONS: Although most of drug hypersensitivity reactions are limited to mild skin conditions, it is important to recognize that even seemingly innocuous medications, such as paracetamol, can cause severe side reactions like Stevens-Johnson syndrome. | Abstract (Spanish): | ANTECEDENTES: El síndrome de Stevens-Johnson es una enfermedad mucocutánea poco frecuente pero potencialmente mortal. Su causa principal es la hipersensibilidad secundaria a la ingesta de fármacos, entre los que destacan carbamazepina, alopurinol y sulfonamidas. El paracetamol, como agente causal, se ha sido reportado poco, con predominio en el continente asiático. CASO CLÍNICO: Paciente masculino de 3 años de edad con antecedente de alergia a penicilina, que inició con máculas eritematovioláceas en el tronco, secundarias a la administración de paracetamol para el alivio de dolor abdominal y testicular. Acudió a urgencias en donde le indicaron antihistamínicos y aumento de la dosis de paracetamol, posterior a lo cual el cuadro se generalizó agregándose ampollas y áreas de piel denudada con afección sistémica. Debido a esto, fue hospitalizado en la unidad de cuidados intensivos pediátricos en donde se estableció el diagnóstico de síndrome de Stevens-Johnson, e inició tratamiento centrado en inmunoglobulina y esteroides con lo que tuvo mejoría clínica progresiva. CONCLUSIONES: Aunque la mayor parte de las reacciones de hipersensibilidad por fármacos se limita a reacciones cutáneas leves, es importante reconocer que aún los medicamentos aparentemente inocuos, como el paracetamol, pueden ser causa de reacciones secundarias severas, como el síndrome de Stevens-Johnson.
