| Abstract (English): | Introduction: Medulloblastoma is the most frequent malignant, invasive primary tumor of the central nervous system in children. It represents approximately 25% of all pediatric intracranial tumors and 33% of all childhood posterior fossa tumors. It is typically located in the posterior fossa presenting as a midline intraaxial mass involving the cerebellar vermis and/or the roof of the fourth ventricle. Its extra-axial location is rare, few cases are reported in the world literature. Objective: To describe a case of medulloblastoma with an infrequent location. Clinical case: This is a 18 years old male patient, who came to the consultation due to headache, gait instability, nausea and vomiting. An extra-axial medulloblastoma in the left cerebellar hemisphere was diagnosed. Total surgical resection of the tumor was performed, with favorable post-surgical evolution. Conclusions: The extra-axial appearance of medulloblastoma is extremely rare, but it should be considered in the differential diagnosis of extra-axial lesions of the posterior fossa. Most extra-axial medulloblastomas occur in the cerebellar pontine angle. However, extra-axial medulloblastomas can also appear in the cerebellar hemisphere, as in this case. [ABSTRACT FROM AUTHOR] |
| Abstract (Spanish): | Introducción: El meduloblastoma es el tumor primario maligno, invasivo, del sistema nervioso central más frecuente en niños. Representa aproximadamente un 25 % de todos los tumores intracraneales pediátricos y un 33 % de todos los tumores de fosa posterior en la infancia. Típicamente se ubica en la fosa posterior presentándose como una masa intraaxial en la línea media que involucra el vermis cerebeloso y/o el techo del cuarto ventrículo. Su ubicación extraaxial es poco frecuente, se reportan pocos casos en la literatura mundial. Objetivo: Describir un caso de meduloblastoma con localización poco frecuente. Caso clínico: Paciente masculino, de 18 años de edad, que acudió a la consulta por cefalea, inestabilidad a la marcha, náuseas y vómitos. Se le diagnosticó un meduloblastoma extraaxial en hemisferio cerebeloso izquierdo. Se le realizó resección quirúrgica total del tumor, con evolución postquirúrgica favorable. Conclusiones: La aparición extraaxial del meduloblastoma es extremadamente rara, pero se debe considerar en el diagnóstico diferencial de las lesiones extraaxiales de fosa posterior. La mayoría de los meduloblastomas extraaxiales ocurren en el ángulo ponto cerebeloso.
