| Abstract (English): | Introduction: iron deficiency is common in patients with heart failure and is also recognized as a risk factor for worsening symptoms, impaired quality of life and a poor prognosis in this population. The objective of this study was to describe the clinical characteristics of patients with heart failure and explore an association between serum iron levels and the New York Heart Association (NYHA) functional classes and left ventricular ejection fraction (LVEF). Method: this was an observational, analytical, cross-sectional study of hospitalized patients from the mideastern integrated healthcare services subnetwork in Bogotá, Colombia. Sociodemographic, clinical and laboratory variables were analyzed using univariate and bivariate analysis and simple logistic regression to calculate the odds ratio (OR). Results: a total of 139 patients hospitalized during the study period yielded a 43% prevalence of heart failure with mildly reduced LVEF; the average age was 53 years and 67.6% were male, with comorbidities like atrial fibrillation (23.2%), coronary disease (59%) and dyslipidemia (27.9%). Altogether, 57.2% of the patients had iron deficiency. The factors associated with NYHA III and IV were male sex (OR: 2.8; 95% CI: 1.17;6.97), chronic obstructive pulmonary disease (OR: 2.7; 95% CI: 1.03;7.23), and iron deficiency (OR: 1.92; 95% CI: 1.13;3.25). Conclusions: most patients with iron deficiency and heart failure had a mildly reduced LVEF, anemia, low ferritin levels and low transferrin saturation. The need to measure the iron kinetics profile of all patients with decompensated heart failure is stressed. The factors associated with NYHA classes III and IV as well as reduced ejection fraction were male sex, chronic obstructive pulmonary disease, hemoglobin < 13 g/dL, and a history of coronary disease. | Abstract (Spanish): | Introducción: la deficiencia de hierro es común en pacientes con insuficiencia cardiaca, siendo reconocido también como un factor de riesgo asociado al empeoramiento de los síntomas, deterioro de la calidad de vida y mal pronóstico en esta población. El objetivo del presente estudio fue describir las características clínicas, y explorar una asociación entre los niveles de hierro sérico y las escalas funcionales New York Heart Association (NYHA) y la fracción de eyección del ventrículo izquierdo (FEVI) en los pacientes con diagnostico con insuficiencia cardiaca. Métodos: estudio observacional analítico de corte transversal de pacientes hospitalizados en la subred Integrada de Servicios de Salud Centro Oriente, Bogotá, Colombia. Se analizaron las variables sociodemográficas, clínicas, de laboratorio, a través de análisis univariado y bivariado y regresión logística simple para calcular Odds ratio (OR). Resultados: un total de 139 pacientes hospitalizados en el periodo estudiado se obtuvo una prevalencia insuficiencia cardiaca con FEVI levemente reducida 43%; la edad promedio fue de 53 años, 67.6% eran hombres, con comorbilidades como fibrilación auricular 23.2%, enfermedad coronaria 59% y dislipidemia 27.9%. El 57.2% pacientes tenían ferropenia. Se hallaron como factores asociados a NYHA III y IV; sexo masculino (OR: 2.8; IC 95%: 1.17;6.97), enfermedad pulmonar obstructiva crónica (OR: 2.7; IC 95%: 1.03;7.23), y ferropenia (OR: 1.92; IC 95%: 1.13;3.25). Conclusiones: en los pacientes con ferropenia y falla cardíaca se identificó que, en su gran mayoría, tenían dentro de sus características una FEVI levemente reducida, anemia, niveles bajos de ferritina y saturación de transferrina. Se recalca la necesidad de medir en todos los pacientes el perfil de ferrocinética, en insuficiencia cardiaca descompensada. Los factores asociados a una clasificación NYHA estadios III y IV, así como a fracción de eyección reducida fueron el sexo masculino, la enfermedad pulmonar obstructiva crónica, una hemoglobina < 13 g/dL y antecedente de enfermedad coronaria.
