| Abstract (English): | OBJECTIVE: To determine the involvement of each component of mucinous carcinomas with micropapillary characteristics, through histopathological study and its relationship with prognosis and treatment. MATERIALS AND METHODS: Comparative, observational, cross-sectional, and secondary source study. All cases were extracted from the database of the Contreras Robledo Pathology Laboratory, of the Dominican Republic, during the period 2015-2023. The study sample met the inclusion criteria, applying a form of 18 questions. Determination of estrogen, progesterone and HER2 receptors was carried out to classify the surrogate type, and EMA/MUC1 for polarity. RESULTS: Micropapillary carcinomas showed inverted polarity: the most frequent were luminal B type, Nottingham grade 2/3, with vascular infiltration in 41% of the cases, and lymphatic infiltration in 64% of the total. Mucinous carcinomas had preserved polarity and most corresponded to the luminal A subtype; Vasculolymphatic infiltration was exceptional. Mucinous carcinomas with micropapillary features showed inverted polarity, but resembled pure mucinous carcinomas, because most cases were of luminal A subtype (90%), with 91% Nottingham grade 1 and vasculolymphatic infiltration being exceptional. CONCLUSION: Mucinous carcinomas with micropapillary characteristics show greater similarity to classic versus micropapillary mucinous carcinomas, according to the surrogate type, Nottingham grade, and vasculolymphatic infiltration. | Abstract (Spanish): | OBJETIVO: Determinar la implicación de cada componente de los carcinomas mucinosos con características micropapilares, mediante el estudio histopatológico y su relación con el pronóstico y tratamiento. MATERIALES Y MÉTODOS: Estudio comparativo, observacional, de corte transversal y de fuente secundaria. Todos los casos fueron extraídos de la base de datos del Laboratorio de Patología Contreras Robledo, de Republica Dominicana, durante el periodo 2015-2023. La muestra de estudio cumplió con los criterios de inclusión, aplicando un formulario de 18 preguntas. Se llevó a cabo la determinación de receptores de estrógenos, progesterona y HER2 para clasificar el tipo subrogado, y EMA/MUC1 para la polaridad. RESULTADOS: Los carcinomas micropapilares mostraron polaridad invertida: los más frecuentes fueron de tipo luminal B, grado 2/3 de Nottingham, con infiltración vascular en el 41% de los casos, e infiltración linfática en el 64% del total. Los carcinomas mucinosos tuvieron polaridad conservada y la mayor parte correspondieron al subtipo luminal A; la infiltración vasculolinfática fue excepcional. Los carcinomas mucinosos con características micropapilares mostraron polaridad invertida, pero se asemejaron a los carcinomas mucinosos puros, porque la mayoría de los casos fueron de subtipo luminal A (90%), con 91% grado 1 de Nottingham e infiltración vasculolinfática es excepcional.
