Abstract (English): The International Association for the Study of Pain defines pain as «an unpleasant sensory and emotional experience associated with, or described in terms of, actual or potential tissue damage». The management of refractory chronic pain is complex. Intrathecal therapy is considered a safe and effective treatment. However, the psychological and social impact also needs to be considered. Ziconotide is an intrathecal drug and a first-line treatment for refractory chronic pain. We present the case of a patient with neuropathic pain after resection of an ependymoma and disc herniation. After failure of pharmacological therapy and spinal cord stimulation, it was decided to implant an intrathecal ziconotide infusion pump. Despite clinical improvement, the patient developed an anxious-depressive disorder and considered euthanizing. The patient was treated with acceptance and commitment therapy, showed improvement in the emotional scale and physical activity, and refused the request for euthanasia. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): La Asociación Internacional para el Estudio del Dolor define el dolor como «una experiencia sensorial y emocional desagradable, asociada a una lesión presente o potencial de los tejidos y expresada en dicho términos». Su abordaje es complejo, pero la terapia intratecal se considera segura y eficaz. La repercusión psicológica y social es un aspecto importante que tratar. La ziconotida es un fármaco de uso intratecal, de primera elección para el dolor crónico refractario. Presentamos el caso de un paciente con dolor neuropático tras la exéresis de un ependimoma y una hernia discal. Tras el fracaso de la terapia farmacológica y de la electroestimulación medular, se colocó una bomba para la administración intratecal de ziconotida. Pese a la mejoría clínica, presentaba un cuadro ansioso-depresivo y se planteaba solicitar la eutanasia. Tras la intervención psicoterapéutica mediante terapia de aceptación y compromiso, el paciente presentó mejoría tanto emocional como física, y rechazó la eutanasia. [ABSTRACT FROM AUTHOR]
