Abstract (English): Introduction. Early childhood caries (ECC) is a public health problem that affects millions of preschool children. The American Academy of Pediatric Dentistry defines ECC as the existence of one or more decayed surfaces on any primary tooth of a child under six years of age. Is a serious form of dental caries with multifactorial origin which significantly affects the child's quality of life. Case report. A five-year-old female patient who went to the emergency room whit non-localized pain. On clinical examination, she presented multiple carious lesions, with clinical crown loss in the upper anterior teeth. It was decided to start the treatment with applications of silver diamine fluoride (SDF) to reduce the bacterial charge, and all teeth are rehabilitated in an integral way recovering the aesthetics and function. Discussion. In the past, the only treatment option for severely decayed teeth was exodontia and replacement with a prosthesis until the permanent teeth erupted. However, with the many techniques and materials currently available, treatment alternatives must be sought, and exodontia given up only as a last resort. Conclusions. The importance of maintaining primary teeth until their natural exfoliation time plays a critical role in maintaining aesthetics, speech development, and the formation of a confident individual. Abstract (Spanish): Introducción. La caries de la primera infancia (ECC) es un problema de salud pública. La Academia Americana de Odontología Pediátrica define la ECC como la existencia de una o más superficies cariadas en cualquier diente primario de un niño menor de seis años. Es una forma grave, con origen multifactorial, que afecta significativamente la calidad de vida del niño. Reporte de caso. Paciente femenino de cinco años de edad, que acudió a consulta de urgencia por presentar dolor no localizado. A la exploración clínica presentó múltiples lesiones cariosas, con pérdida de corona clínica en los dientes anteriores superiores. Se realizó tratamiento con aplicaciones de fluoruro diamino de plata (FDP) para disminuir la carga bacteriana, y se rehabilitó de forma integral recuperando la estética y función. Discusión. En el pasado, la única opción de tratamiento para los dientes con lesiones severas de caries era la exodoncia y reemplazo con una prótesis hasta que erupcionaran los dientes permanentes. Sin embargo, con las numerosas técnicas y materiales disponibles actualmente, se deben buscar alternativas de tratamiento y desistir de la exodoncia solo como último recurso. Conclusiones. La importancia de mantener los dientes primarios hasta su tiempo de exfoliación natural desempeña un papel fundamental en el mantenimiento de la estética, el desarrollo del habla y la formación de un individuo seguro de sí mismo.
