Abstract (English): Introduction: Papillary thyroid cancer is common and has a high recurrence rate of about 20%. Adequate treatment, including surgery and radioiodine, is crucial for a good prognosis. Staging to evaluate cervical lymph node metastasis is essential before surgery. Diagnostic methods such as fine-needle aspiration biopsy (FNAB) guided by ultrasound and thyroglobulin measurement are useful but can present false negatives, affecting the prognosis. Objective: To evaluate the correlation between the level of thyroglobulin from aspiration and the disease found in the histology of the lymph nodes. Materials and Methods: Patients with thyroid cancer operated at the Clinical Hospital of the University of Chile between 2012 and 2022 are described. These patients underwent aspiration and thyroglobulin measurement in suspicious lymph nodes. The thyroglobulin value was correlated with the histological result of the lymph nodes. Results: A total of 51 patients were included. The average thyroglobulin level was 298 ng/ml, with 363 ng/ml in those with a positive FNAB and 184 ng/ml in those with a negative FNAB. 39 patients underwent dissection, and their average FNAB thyroglobulin level was 359,02 ng/ml. Discussion: There is a statistically significant association between FNAB thyroglobulin levels and FNAB results (p-value = 0.010). Among patients with FNAB thyroglobulin levels < 100 ng/ml, about 30% had a positive FNAB, compared to 77% of those with FNAB thyroglobulin levels > 400 ng/ml. Conclusion: It is important to perform thyroglobulin measurement in FNAB to complement the diagnosis of cervical metastasis in all patients with suspicious lymph nodes. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción: El cáncer de tiroides papilar es común y tiene una alta tasa de recurrencia, cerca del 20%. El tratamiento adecuado, que incluye cirugía y radioyodo, es crucial para un buen pronóstico. La etapificación para evaluar metástasis ganglionares cervicales es esencial antes de la cirugía. Métodos diagnósticos como la PAAF guiada por ecografía y la medición de tiroglobulina son útiles, pero pueden presentar falsos negativos que afectan el pronóstico. Objetivo: Evaluar la correlación entre el nivel de tiroglobulina de la punción y la enfermedad encontrada en la histología de las adenopatías. Materiales y Métodos: Se describe los pacientes con cáncer de tiroides operados en el nuestro hospital entre 2012 y 2022. Estos pacientes se sometieron a punción y medición de tiroglobulina en la adenopatía sospechosa. Se correlacionó el valor de la tiroglobulina con el resultado histológico de las adenopatías. Resultados: 51 pacientes en total. La tiroglobulina promedio fue de 298 ng/ml, mientras que en los con PAAF positiva fue de 363 ng/ ml y en los con PAAF negativa fue de 184 ng/ml. 39 pacientes fueron a disección, estos tenían en promedio una Tg PAAF de 359,02 ng/ml. Discusión: Existe una asociación estadísticamente significativa entre los niveles de TgPAAF y PAAF (p-value = 0,010), en pacientes con TgPAAF < 100, cerca de un 30% tiene PAAF positiva, versus el 77% de los con TgPAAF > 400. Conclusión: Es importante realizar el estudio de tiroglobulina en punción para complementar el diagnóstico de metástasis cervicales en todos los pacientes con adenopatías sospechosas.
