| Abstract (English): | Aim: In this analysis we will assess psychological distress at diagnosis of advanced cancer, perceived dignity and its relationship with sociodemographic, clinical, and psychological factors and coping strategies as modulators between uncertainty and psychological distress. Method: This study was based on the NEOetic project, a prospective, multicentre registry promoted by the Bioethics section of the Spanish Society of Medical Oncology (SEOM). Several scales were used: BSI-18, PPDS, MUIS, DUFSSQ and MINI-MAC. In addition, sociodemographic and clinical aspects and their relationship with the other variables were analyzed. Results: Patients treated during the Covid-19 pandemic exhibited high levels of anxiety and depression, particularly associated with young age at diagnosis, life expectancy over 18 months, female sex, cancer-related concerns, and hopelessness. Patients with low perceived dignity experienced higher levels of anxiety and depression, along with lower positive attitudes. Psychological distress affected 72% of patients, with positive attitude and cognitive avoidance being the most employed coping strategies. Conclusions: These findings underscore the importance of addressing the mental health of cancer patients, especially during crises like the pandemic, by promoting effective coping strategies and providing appropriate emotional support. Oncology professionals are encouraged to conduct regular assessments of psychological well-being and tailor specific interventions based on identified factors to enhance emotional resilience and improve patients' quality of life. | Abstract (Spanish): | Objetivo: en este análisis evaluaremos el malestar psicológico al diagnóstico del cáncer avanzado, la dignidad percibida y su relación con factores sociodemográficos, clínicos y psicológicos, así como las estrategias de afrontamiento como moduladoras entre la incertidumbre y el malestar psicológico. Método: este estudio se fundamentó en el proyecto NEOetic, un registro prospectivo y multicéntrico promovido por la sección de Bioética de la Sociedad Española de Oncología Médica (SEOM). Se utilizaron diversas escalas: BSI-18, PPDS, MUIS, DUFSSQ y MINI-MAC. Además, se analizaron factores sociodemográficos y clínicos y su relación con el resto de variables. Resultado: los pacientes tratados durante la pandemia de Covid-19 mostraban altos niveles de ansiedad y depresión, especialmente vinculados a ser joven al diagnóstico, tener una expectativa de vida superior a 18 meses: al sexo femenino, tener preocupación por el cáncer y experimentar desesperanza. Aquellos con bajos niveles de dignidad percibida mostraron mayor ansiedad y depresión, y una actitud menos positiva. El malestar psicológico afectó al 72% de los pacientes, siendo la actitud positiva y la evitación cognitiva las estrategias de afrontamiento más empleadas. Conclusiones: estos hallazgos subrayan la importancia de abordar la salud mental de los pacientes oncológicos especialmente en situaciones de crisis como la pandemia. Es esencial promover estrategias de afrontamiento eficaces y proporcionar un apoyo emocional adecuado. Se recomienda a los oncólogos realizar evaluaciones periódicas del estado psicológico de sus pacientes, adaptando intervenciones personalizadas que respondan a los factores identificados para reforzar la resiliencia emocional y mejorar la calidad de vida de los pacientes.
