| Abstract (English): | Background and objective. Reconstruction of the perineal area must consider not only providing stable coverage with an acceptable aesthetic result, but also allowing functional recovery and improving the patient's quality of life. We present our experience in the use of Ubiquitous Nanoperforator Aleatoric Locoregional flaps ((UNAL) for the reconstruction of defects in this area. Methods. We present a series of cases obtained from the Plastic Surgery Service of the National University of Colombia from 2012 to 2021 about reconstruction of defects in the perineal region secondary to different etiologies under UNAL flap. In addition, we seek to compare the results obtained with those observed in the literature. Results. UNAL flap was performed in 14 patients. The most frequent etiology was Fournier's gangrene, 2 patients presented major complications that required reintervention, 1 of them suffered complete flap loss requiring the design of another type of flap and 10 presented minor complications being dehiscence the most common of them. Twelve patients achieved sedation in the first month and standing at 2 months, in 2 patients it was not possible to evaluate this result because they had neurological diseases that did not allow mobility of the lower limbs or the trunk. All patients had a minimum follow-up of 6 months. Conclusions. In our experience, the UNAL flap is established as a safe, easily reproducible reconstructive option, with a low rate of morbidity of the donor site, low rate of failure in reconstruction, stable coverage, allowing early rehabilitation, and a rate of minor complications that can be easily managed, at low cost, and with adequate long-term results. La reconstrucción del periné busca proveer una cobertura estable, con resultados estéticos aceptables y la mejor recuperación funcional posible para mejorar la calidad de vida del paciente. Presentamos nuestra experiencia con el uso de colgajos locorregionales de nanoperforantes aleatorias de estilo libre para la reconstrucción de defectos en esta zona. Material y método. Serie de casos resultado de la experiencia del Servicio de Cirugía Plástica de la Universidad Nacional de Colombia desde 2012 a 2021 en reconstrucción de defectos de la región perineal secundarios a diferentes etiologías con el uso de colgajos locorregionales de nanoperforantes aleatorias de estilo libre (UNAL por sus siglas en inglé Ubiquitous Nanoperforator Aleatoric Locoregional flaps). Adicionalmente, comparamos los resultados obtenidos con lo observado en la literatura. Resultados. Empleamos el colgajo tipo UNAL en 14 pacientes. La etiología más frecuente fue la gangrena de Fournier, 2 pacientes presentaron complicaciones mayores que requirieron reintervención, 1 de ellos sufrió perdida completa del colgajo requiriendo el diseño de otro tipo de colgajo, y 10 presentaron complicaciones menores, siendo la dehiscencia la más común de ellas. En 12 se logró la sedestación en el primer mes y la bipedestación a los 2 meses; en 2 no fue posible la valoración de este resultado por presentar enfermedades neurológicas que no permitían la movilidad de los miembros inferiores o del tronco. Todos los pacientes tuvieron un seguimiento mínimo de 6 meses. Conclusiones. En nuestra experiencia, el colgajo UNAL resultó ser una opción reconstructiva segura, estable y fácilmente reproducible, con escasa morbilidad del sitio donante, alta tasa de éxito en la reconstrucción y que permite la rehabilitación temprana con una tasa de complicaciones menores que pueden ser manejadas fácilmente, con bajo costo y con adecuados resultados a largo plazo.
