| Abstract (English): | A 73-year-old, dependent, polymedicated, multi-pathological patient contacts the Community Pharmacy by telephone to request information on a newly prescribed treatment for a urinary tract infection and vitamin D deficiency. The dispensing of the medication to her caregiver leads to further doubts for the patient, so she is offered the Medication Use Review (MUR) service. During the service, several incidences are detected in her treatment, so it is decided to make a referral report to the Primary Care Physician for a complete review of the treatment and thus be able to refer the patient to the Personalized Dosage System (SPD) service in order to guarantee the patient’s pharmacotherapeutic safety and improve her adherence to the treatment. In this case, the provision of the RUM service served as a gateway to the SPD and made it possible to detect Medication Related Problems (MRP) that could lead to therapeutic failure. | Abstract (Spanish): | Paciente 73 años, dependiente, polimedicada y pluripatológica contacta telefónicamente con la farmacia comunitaria para solicitar información sobre un tratamiento de nueva prescripción para una infección de orina y un déficit de vitamina D. La dispensación de la medicación a su cuidadora deriva en mayores dudas a la paciente, por lo que se le ofrece el servicio de revisión del uso del medicamento (RUM). Durante la prestación del servicio se detectan varias incidencias en su tratamiento, por lo que se decide realizar un informe de derivación al médico de atención primaria para una revisión completa del tratamiento y poder así derivar a la paciente al servicio de Sistema Personalizado de Dosificación (SPD) con el fin de garantizar la seguridad farmacoterapéutica de la paciente y mejorar su adherencia al tratamiento. En este caso, la prestación del servicio de RUM sirvió de pasarela al SPD y permitió detectar problemas relacionados con la medicación (PRM) que podían conducir a un fracaso terapéutico.
