Abstract (English): This article presents a teaching proposal with a social constructivist model conceptualizing the innovations in internal medicine residents’ practical training for treating ambulatory patients. This proposal is illustrated with type 2 diabetes as a prevalent disease. This breakthrough arises from the author’s journey as an internist and educator, especially with the study “Construction and evaluation of a professional skill profile in internal medicine.” The article discusses the general and specific objectives, justification, teaching proposal with didactic strategies and actions, type 2 diabetes line of research, concept of professional skills, teaching model and B-learning, practical training in the outpatient department, opportunities for improvement and final thoughts. Professors and program coordinators are invited to reflect on and review the time devoted to ambulatory patient care and service during residency, as well as the development of medical residents’ professional skills. Abstract (Spanish): Se presenta una propuesta pedagógica con modelo socio-constructivista al conceptualizar las innovaciones en el desarrollo de la práctica formativa de médicos residentes en medicina interna, para atender a pacientes ambulatorios. Esta propuesta se ejemplifica con la diabetes tipo 2 como enfermedad prevalente. Este avance surge del camino recorrido por la autora, internista y educadora, en particular con la investigación “Construcción y evaluación de un perfil de competencias profesionales en medicina interna”. Se exponen los objetivos general y específicos, justificación, la propuesta pedagógica con acciones y estrategias didácticas, la línea de investigación en diabetes tipo 2, concepto de competencias profesionales, modelo pedagógico y B-Learning, la práctica formativa en consulta externa y B-Learning, opciones de mejora y reflexiones finales. Se invita a profesores y coordinadores de programa a reflexionar y revisar el tiempo dedicado a la atención y servicio a pacientes ambulatorios durante la residencia, así como el desarrollo de competencias profesionales por los médicos residentes.
