| Abstract (English): | A 71-year-old male patient with a long-standing history of diffuse brown pigmentation on the hands. Six months before consultation, he developed predominantly left-sided numbness, affecting finger extension and the medial aspect of the wrist, without visible skin lesions. Pure neural leprosy was suspected. Thickening of the left ulnar nerve was noted, prompting electromyography, which revealed reduced conduction velocity, initially interpreted as compressive neuropathy. A biopsy from the dorsum of the left hand showed a dermal hemangioma with an accompanying by lymphocytic infiltrate containing acid-fast bacilli. A diagnosis of hemangioma and pure neural multibacillary leprosy was established. Multidrug therapy was initiated, and the patient is now considered cured of Hansen's disease. This case highlights the significance of angiomatous lesions and hand numbness, even in the absence of overt skin lesions. The clinicopathological correlation enabled a detailed review of the neural alterations observed in the biopsy, ultimately confirming the diagnosis of pure neural leprosy. [ABSTRACT FROM AUTHOR] |
| Abstract (Spanish): | Un paciente de 71 años, con un cuadro de larga data de pigmentación parda difusa en las manos. Seis meses antes de la consulta presenta adormecimiento de predominio izquierdo, con extensión a los dedos y el borde interno de la muñeca. Sin lesiones cutáneas. Se sospechó lepra neural pura. Se detectó engrosamiento del nervio cubital izquierdo, que requirió electromiografía, la cual demostró disminución de la velocidad de conducción, interpretada como de origen compresivo. La biopsia del dorso de la mano izquierda demostró hemangioma dérmico acompañado de infiltrado linfocitario con bacilos ácido-alcohol resistentes. Se diagnosticó hemangioma y lepra multibacilar neural pura. Se inició tratamiento con poliquimioterapia (PQT). Actualmente, se considera curado de la enfermedad de Hansen. La importancia radica en la prominencia de la reacción angiomatosa y el entumecimiento de las manos, aunque no hubiera lesiones cutáneas. La correlación clinicopatológica permitió revisar los cambios neurales de la biopsia y confirmar el diagnóstico de lepra neural pura. [ABSTRACT FROM AUTHOR] |
