Abstract (English): Introduction: The incidence of cardiac arrest in the United Kingdom is of 55 per 100.000 inhabitants. Of these, only one of every ten patients has access to a defibrillator and shockable rhythm is identified in one of every four patients. Defibrillation is one of the key links in the management of both basic and advanced resuscitation. Objective: This scoping review aims at reviewing the literature focusing on the effectiveness on selfadhesive patches in electrical therapy for cardiac arrest and the electrical instability in the adult hospital environment. Methods: Searches were conducted in Medline and Embase until January, 2024, including primary trials (excluding case series and animal studies) and systematic reviews. The participants were hospitalized adult patients requiring electrical cardioversion or defibrillation. The intervention assessed involved the use of self-adhesive patches versus paddle electrodes, to measure effectiveness and safety. The searches were conducted using controlled and non-controlled terms, excluding grey literature. Two of the authors independently reviewed the titles and abstracts and settled any disagreements by consensus. Relevant data were extracted and synthetized on one table for an overall qualitative summary of the outcomes. Results: Ten references were identified in the literature search. After ruling out duplicates, nine references were reviewed based on title and abstract. Three studies met the inclusion criteria and the complete text was screened. No studies were found which conclusively concluded the effectiveness of self-adhesive patches used in electrical therapy for hospital resuscitation. The studies included fail to discuss safety results of the interventions. Conclusion: When mapping the topics of intervention, this scoping review has identified the need for further systematic research on the effectiveness of self-adhesive patches for electrical cardioversion and defibrillation in hospital settings. Notwithstanding the evidence supporting electrical therapy to revert unstable tachyarrhythmias and ventricular fibrillation/pulseless ventricular tachycardia, further studies are needed comparing paddle electrodes versus self-adhesive patches in the hospital setting. Future prospective trials are required to establish the comparative effectiveness of these methods in the hospital management of cardioversion/defibrillation. [ABSTRACT FROM AUTHOR]

De estos, solo uno de cada diez pacientes tiene acceso a un desfibrilador y se encuentra ritmo desfibrilable en uno de cada cuatro pacientes. La desfibrilación es uno de los eslabones clave tanto en el manejo básico como en el manejo avanzado de la reanimación. Objetivo: Esta revisión de alcance tiene como objetivo revisar la literatura que investiga la efectividad de los parches autoadhesivos en la terapia eléctrica para el paro cardiaco y la inestabilidad eléctrica en el entorno hospitalario en pacientes adultos. Métodos: Se realizaron búsquedas en Medline y Embase hasta enero de 2024, incluyendo estudios primarios (excluyendo series de casos y estudios en animales) y revisiones sistemáticas. Los participantes eran pacientes adultos hospitalizados que requerían cardioversión eléctrica o desfibrilación. La intervención evaluada fue la de uso de parches autoadhesivos frente a palas, para medir la efectividad y la seguridad. Las búsquedas se realizaron utilizando términos controlados y no controlados, excluyendo literatura gris. Dos autores revisaron los títulos y resúmenes de forma independiente y resolvieron desacuerdos por consenso. Se extrajeron datos relevantes y se sintetizaron en una tabla, para hacer una síntesis cualitativa global de los desenlaces. Resultados: En la búsqueda de literatura se identificaron diez referencias. Tras eliminar duplicados, se revisaron nueve referencias en título y resumen; resultaron tres estudios que cumplían los criterios de inclusión y fueron tamizados en texto completo. No se encontraron estudios que concluyeran de forma contundente la efectividad del uso de los parches autoadhesivos en la terapia eléctrica de la reanimación intrahospitalaria. Los estudios incluidos no presentaban resultados de seguridad de las intervenciones. Conclusión: Al mapear los temas de intervención, esta revisión de alcance ha identificado la necesidad de una mayor investigación sistemática sobre la efectividad de los parches autoadhesivos para la cardioversión eléctrica y la desfibrilación en entornos hospitalarios. A pesar de la evidencia que respalda la terapia eléctrica para revertir las taquiarritmias inestables y la fibrilación ventricular/taquicardia ventricular sin pulso, faltan estudios que comparen los electrodos de palas con los parches autoadhesivos en entornos hospitalarios. Se necesitan estudios prospectivos futuros para determinar la efectividad comparativa de estos métodos en el manejo hospitalario de la cardioversión/desfibrilación. [ABSTRACT FROM AUTHOR]
