| Abstract (English): | Introduction and goals: Impella® is a left ventricular assist device that can be used to facilitate high-risk coronary angioplasty, in the treatment of cardiogenic shock, high-risk percutaneous coronary intervention, and as a bridge to heart transplantation. The objective of this case was to establish a nursing care plan for patients carrying the Impella ® 5.5 ventricular assist device, implanted transaxillary. Material and Methods: A nursing assessment was carried out following the Virginia Henderson 14 needs model during the early mobilization of the patient. A care plan was drawn up using NANDA-NOC-NIC taxonomy. The results were evaluated with a Likert scale. Results and conclusions: We highlight the following diagnoses: Impaired physical mobility; Anxiety; Ineffective health self-management; Risk of accidental decannulation, bending or kinking, and Risk of decreased cardiac output. Risk of Infection and Risk of bleeding were the main collaboration problems. The nursing role is key to promoting the safe and early mobilization of these patients, helping to prevent and reduce complications associated with prolonged bed rest. [ABSTRACT FROM AUTHOR] |
| Abstract (Spanish): | Introducción y objetivos: el Impella® es un dispositivo de asistencia ventricular izquierda que se puede utilizar como asistencia durante la angioplastia coronaria de alto riesgo, en el tratamiento del shock cardiogénico y como terapia puente a trasplante cardíaco. El objetivo del presente caso es establecer un plan de cuidados de enfermería para los pacientes portadores de asistencia ventricular Impella® 5.5 implantada vía transaxilar en la movilización y la sedestación precoz. Material y métodos: se realizó una valoración enfermera según las 14 necesidades de Virginia Henderson para la movilización precoz del paciente y se elaboró un plan de cuidados utilizando la taxonomía NANDA-NOC-NIC, se evaluaron según la escala Likert. Resultados y conclusiones: se destacaron los diagnósticos: deterioro de la movilidad física, ansiedad, gestión ineficaz de la propia salud, riesgo de lesión por decanulación accidental, acodaduras y riesgo de disminución del gasto cardíaco. Como problemas de colaboración se destacaron el riesgo de infección y de sangrado. El papel de enfermería es clave para promover la movilización segura y precoz de estos pacientes, ayudando a prevenir y reducir las complicaciones asociadas al encamamiento prolongado.
