| Abstract (English): | Introduction: Local recurrence after treatment of rectal cancer (RC) has decreased from 40% to 4-10% thanks to total mesorectal excision and neoadjuvant therapy. Between 7% and 25% of patients may have neoplastic involvement in the lymph nodes of the lateral pelvic compartments, contributing to local recurrence. Surgical technique of lateral pelvic lymphadenectomy (LPL) aims to resect these lymph nodes. The main objective of this study is to evaluate the perioperative results of the implementation of this technique in a Public Hospital. Material and Methods: A concurrent cohort study was conducted that included patients with RC undergoing LPL with curative intent. The primary objective is to evaluate perioperative morbidity, and the secondary objective is to evaluate pathological results. Demographic, preoperative, postoperative and oncological variables were analyzed using descriptive statistics. Results: 5 LPLs were performed. Average age was 56 years. Four patients had suspected obturator involvement and 1 internal iliac. Surgical approach was laparoscopic in 4 cases, with 1 conversion due to bleeding. The average LPL time was 117 min. Two patients had metastatic lymph nodes. There was 1 reoperation for anastomotic leak. No serious nerve injuries or need for permanent urinary catheter were recorded. | Abstract (Spanish): | Introducción: La recurrencia local luego del tratamiento del cáncer de recto (CR) ha disminuido del 40% al 4 a 10% gracias a escisión total del mesorrecto y la neoadyuvancia. Entre 7 y 25% de los pacientes podrían presentar compromiso neoplásico en los linfonodos de los compartimentos laterales pélvicos, contribuyendo a la recurrencia local. La técnica quirúrgica linfadenectomía lateral pélvica (LNLP) tiene como objetivo la resección de estos linfonodos. Este estudio tiene como objetivo principal evaluar los resultados perioperatorio de la implementación de esta técnica en un Hospital Público. Material y Métodos: Se realizó un estudio de cohorte concurrente que incluyó a pacientes con CR sometidos a LNLP con intención curativa. El objetivo primario es evaluar morbilidad perioperatoria y el objetivo secundario los resultados patológicos. Se analizaron variables demográficas, preoperatorias, posoperatorias y oncológicas mediante estadística descriptiva. Resultados: Se realizaron 5 LNLP. La edad promedio de 56 años. Cuatro pacientes presentaban sospecha de compromiso obturatriz y 1 iliaco interno. El abordaje quirúrgico fue laparoscópico en 4 casos, con 1 conversión por sangrado. El tiempo promedio de LNLP fue de 117 minutos. Dos pacientes presentaron linfonodos metastásicos. Hubo 1 reintervención por filtración anastomótica. No se registraron lesiones nerviosas graves ni necesidad de sonda urinaria a permanencia. Conclusión: Nuestra serie evidencia que la implementación de la LNLP es factible para un equipo quirúrgico experimentado. [ABSTRACT FROM AUTHOR] |
