Abstract (English): seconds, without concomitant hearing loss, and associated with head movements. In some cases, this disease does not present nystagmus during the Dix-Hallpike maneuver due to fatigue, a small number of otoconia, or improper execution of the maneuver; this is referred to as subjective benign paroxysmal positional vertigo. OBJECTIVE: To describe subjective benign paroxysmal positional vertigo through a literature review. METHODOLOGY: A narrative literature review was conducted covering the period from 2000 to 2023, using the terms subjective and benign paroxysmal positional vertigo. The search engines used were Google Scholar and PubMed. Priority was given to case-control studies comparing patients with objective vs subjective benign paroxysmal positional vertigo. Clinical and epidemiological variables, as well as treatment and success rates, were analyzed. RESULTS: Subjective benign paroxysmal positional vertigo accounts for 11.5% to 48% of total vertigo cases. Both types share similar clinical characteristics and the same treatment approach. Various authors have found that the effectiveness rate of vertigo resolution (whether partial or complete recovery) is similar between objective and subjective benign paroxysmal positional vertigo. CONCLUSIONS: It is emphasized the need to include subjective benign paroxysmal positional vertigo in the diagnostic suspicion and to recommend particle repositioning maneuvers, which have the same effectiveness rate. Abstract (Spanish): ANTECEDENTES: El vértigo postural paroxístico benigno se caracteriza por episodios de vértigo de segundos de duración, sin hipoacusia concomitante, asociado con los movimientos de la cabeza. Este padecimiento en ocasiones no muestra nistagmo a la maniobra de Dix-Hallpike debido a agotamiento, poca cantidad otoconial o inadecuada práctica de la maniobra, a esto se le llama vértigo postural paroxístico benigno subjetivo. OBJETIVO: Describir el vértigo postural paroxístico benigno subjetivo a través de una revisión de la bibliografía. METODOLOGÍA: Revisión narrativa de la bibliografía de 2000 a 2023. Se buscaron los términos: subjetivo y vértigo postural paroxístico benigno. Los buscadores utilizados fueron Google Académico y PubMed. Se priorizaron los estudios de casos y controles que comparaban pacientes con vértigo postural paroxístico objetivo vs subjetivo. Se analizaron las variables clínicas, epidemiológicas, el tratamiento y el porcentaje de éxito. RESULTADOS: El vértigo postural paroxístico benigno subjetivo representa entre el 11.5 y el 48% del total de los casos de vértigo. Ambos tipos muestran características clínicas similares y tienen el mismo tratamiento. Diversos autores han encontrado una tasa de efectividad de eliminación del vértigo (ya sea recuperación parcial o completa) similar entre vértigo postural paroxístico benigno objetivo y subjetivo. CONCLUSIONES: Se insiste en la necesidad de incluir en la sospecha diagnóstica el vértigo postural paroxístico benigno subjetivo, así como recomendar las maniobras de reposicionamiento de partículas, que tienen la misma tasa de efectividad, además de ser un procedimiento de bajo costo y fácil de realizar.
