Abstract (English): Women have a higher prevalence of chronic diseases, especially musculoskeletal conditions or diseases more characteristic of women that cause pain. They also more frequently experience predisposing factors for the development of chronic pain (obesity, anxiety or depressive disorders, sleep disturbance, or interpersonal violence). Despite these facts, chronic pain is greater and more prevalent in women, even in the presence of the same disease and risk factors. This article explores the differential factors that determine the processing of pain between females and males, between women and men. It highlights how women detect nociceptive stimuli earlier, respond with greater intensity, use less automatic control, and process pain more slowly—engaging brain structures especially related to emotional processing and memory. These differences, which underline a distinct approach to threats and potential injuries, also begin in the interactions between the immune system and neurons during the development of injuries, as the cellular and molecular signaling mechanisms differ between women and men, opening the door to a better understanding of this relationship between pain and women, and facilitating a more specific therapeutic approach. Abstract (Spanish): La mujer presenta una mayor prevalencia de enfermedades crónicas, especialmente musculoesqueléticas o enfermedades más características de la mujer que desarrollan dolor. También padece, más frecuentemente, factores predisponentes para el desarrollo de dolor crónico (obesidad, trastornos de ansiedad o depresión, alteración del sueño o violencia interpersonal). Pero, a pesar de estos hechos, el dolor crónico es mayor y más prevalente en la mujer, incluso en presencia de la misma enfermedad y los mismos factores de riesgo. Este artículo explora los hechos diferenciales que determinan el procesamiento del dolor entre las hembras y los machos, entre las mujeres y los hombres. Se destaca cómo la mujer detecta antes el estímulo nociceptivo, responde con una mayor intensidad, utiliza menos el control automático del mismo y lo procesa más pausadamente, utilizando estructuras cerebrales especialmente relacionadas con el procesamiento emocional y la memoria. Estas diferencias, que subrayan un abordaje diferencial ante las amenazas y potenciales lesiones, se inician también en las interacciones que se producen entre el sistema inmunológico y las neuronas en el desarrollo de las lesiones, siendo los mecanismos de señalización celular y molecular distintos entre mujeres y hombres, lo que abre la puerta a la comprensión de esta relación entre el dolor y la mujer, y facilita un abordaje terapéutico más específico.
