Abstract (English): Introduction: medical history increases the risk of fragility fractures, predisposing to more vulnerable areas according to the type of history. Objective: to determine the most vulnerable areas of fragility fractures according to clinical history. Materials and method: analytical and crosssectional study of a secondary database of 719 adults. The variables were the presence of shoulder, wrist, hip and ankle fractures, age, sex, diabetes mellitus, hypertension, obesity, smoking, reduced mobility, dyslipidemia, osteoporosis, age, arthritis, renal, cardiovascular and thyroid disease, cancer and number of comorbidities. The automatic detection tree of chi-square interactions and Cramer's V were used. Results: the decision tree identified key variables of fragility fracture areas, age group, diabetes and comorbidities. Ankle fractures were associated primarily with individuals aged 50 to 65 years with diabetes (V=0.181; p<0.001). Hip fractures with individuals aged 84 years or older (V=0.332; p<0.001). Shoulder and wrist fractures with individuals aged 66 to 83 years with two or fewer comorbidities (V=0.216; p<0.001). Conclusions: fragility fractures are significantly associated with age and the presence of comorbidities, especially diabetes. People older than 84 years have a greater propensity for hip fractures, those between 50 and 65 years with diabetes are more prone to ankle fractures. Shoulder and wrist fractures are more common in individuals aged 66 to 83 years with few comorbidities. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción: los antecedentes clínicos aumentan el riesgo de fracturas por fragilidad, predisponiendo a zonas más vulnerables según el tipo de antecedente. Objetivo: determinar las zonas de fracturas por fragilidad más vulnerables según antecedentes clínicos. Materiales y método: estudio analítico y transversal de una base de datos secundaria de 719 adultos. Las variables fueron la presencia de fracturas de hombro, muñeca, cadera y tobillo, edad, sexo, diabetes mellitus, hipertensión, obesidad, fumador, movilidad reducida, dislipidemia, osteoporosis, edad, artritis, enfermedad renal, cardiovascular y tiroidea, cáncer y número de comorbilidades. Se usó el árbol de detección automático de interacciones de chi-cuadrado y V de Cramer. Resultados: el árbol de decisiones identificó variables clave de áreas de fracturas por fragilidad, al grupo etario, diabetes y comorbilidades. Las fracturas de tobillo se asociaron principalmente con personas de 50 a 65 años con diabetes (V=0,181;p<0,001). Las fracturas de cadera con personas de 84 años o más (V=0,332;p<0,001). Las fracturas de hombro y muñeca con individuos de 66 a 83 años con dos o menos comorbilidades (V=0,216;p<0,001). Conclusiones: las fracturas por fragilidad están significativamente asociadas con la edad y la presencia de comorbilidades, especialmente la diabetes. Las personas mayores de 84 años tienen mayor propensión a fracturas de cadera, aquellos entre 50 y 65 años con diabetes son más propensos a fracturas de tobillo.
