Abstract (English): Recent advancements in techniques applied to the study of bone marrow histoarchitecture and the iden tification of genetic, epigenetic, proteomic, and meta bolic characteristics of hematopoietic progenitor cells have facilitated a more comprehensive understanding of the interaction between these cells and the marrow microenvironment, as well as the mechanism of hema topoiesis. Hematopoiesis is maintained by primitive pro genitor cells or stem cells, which undergo asymmetric division, enabling self-renewal. Pluripotent progenitor cells, which also possess self-renewal capabilities, are responsible for sustaining hematopoiesis. Hematopoi etic progenitor cells represent a population in con tinuous and progressive differentiation towards mature cells, exhibiting plasticity to modify their proliferation and differentiation in response to received signals. In acute or chronic infections or inflammation, inflam matory cytokines and/or extracellular vesicles induce genetic, epigenetic, proteomic, and metabolic changes in hematopoietic progenitor cells, initiating emergency hematopoiesis. Emergency hematopoiesis constitutes an alteration of hematopoiesis characterized by 1) in creased myelopoiesis, with decreased lymphopoiesis and erythropoiesis, 2) increased release of hematopoietic progenitor cells into peripheral blood and development of extramedullary hemopoiesis, 3) generation of two cellular subpopulations of the innate immune system with defined characteristics and functions a) innate im mune memory cells (trained immunity), and b) myeloid suppressor cells. These changes can induce alterations in the neutrophil/lymphocyte and monocyte/lymphocyte ratio in the blood count, the increase of which has ad verse prognostic value in various chronic inflammatory diseases and especially in neoplasms. Abstract (Spanish): Nuevas técnicas aplicadas al estudio de la histo-arquitectura de la médula ósea y a la identificación y características genéticas, epigenéticas, proteómicas y metabólicas de las células progenitoras hematopo yéticas, han permitido conocer mejor la interacción entre estas células y el microambiente medular y el mecanismo de la hematopoyesis. La hematopoyesis es mantenida por las células progenitoras primitivas o stem cells, que tienen una división asimétrica posi bilitando la autorrenovación. Las células progenitoras pluripotenciales, también autorrenovables, son las res ponsables de mantener la hematopoyesis. Las células progenitoras hematopoyéticas constituyen células en continua y progresiva diferenciación hacia células ma duras, con plasticidad para modificar su proliferación y diferenciación, dependiendo de las señales que reci ban. En infecciones o inflamación aguda o crónica, las citoquinas inflamatorias y/o vesículas extracelulares, provocan cambios genéticos, epigenéticos, proteómicos y metabólicos en las células progenitoras hematopoyéti cas, induciendo una "hematopoyesis de emergencia". La hematopoyesis de emergencia constituye una alteración de la hematopoyesis caracterizada por 1) aumento de la mielopoyesis, con disminución de la linfopoyesis y eritropoyesis, 2) aumento de la liberación de células progenitoras hematopoyéticas a sangre periférica y ge neración de hemopoyesis extramedular, 3) generación de dos subpoblaciones celulares del sistema inmune innato con características y funciones definidas a) células de memoria inmune innata o inmunidad trained y b) células mieloides supresoras. Como consecuencia de estos cambios, en el hemograma se detectan alteraciones en la relación neutrófilo/linfocito y monocito/linfocito, cuyo aumento tiene valor pronóstico adverso en diversas enfermedades inflamatorias crónicas y especialmente, en neoplasias.
