| Abstract (English): | This case report reviews the phenomenon of rebound pain (RP) following peripheral nerve blocks (PNB), a prevalent issue in regional anesthesia. RP is defined as intense pain that occurs when the effect of the PNB wears off, often exceeding the initial pain. Risk factors, pathophysiological mechanisms, and mitigation strategies are analyzed. It is highlighted that RP intensity may depend on the duration of the block, the technique used, and individual patient characteristics. Preventive strategies include multimodal analgesia, prophylactic dexamethasone use, and continuous PNB, as well as patient education. The importance of future studies is also discussed to better understand this phenomenon and develop more effective strategies for its prevention (especially in patients with chronic pain), its diagnosis and its treatment. | Abstract (Spanish): | En este caso se revisa el fenómeno del rebound pain (RP) tras el bloqueo de nervios periféricos (BNP), un problema que podría estar afectando hasta un 50% de los pacientes con anestesia regional. El RP se define como un dolor intenso que aparece cuando el efecto del BNP desaparece, frecuentemente de mayor intensidad que el dolor inicial. Se analizan factores de riesgo, mecanismos fisiopatológicos y estrategias de mitigación. Se destaca que la intensidad del RP puede depender de la duración del bloqueo, la técnica empleada y las características individuales del paciente. Entre las estrategias preventivas, se mencionan la analgesia multimodal, el uso de dexametasona profiláctica y el BNP continuo, así como educación al paciente sobre el dolor. También se discute la importancia de estudios futuros para comprender mejor este fenómeno y desarrollar estrategias más efectivas para su prevención (especialmente en pacientes con dolor crónico), su diagnóstico y su tratamiento.
