Abstract (English): OBJETIVE To present a surgical approach to gender reassignment (vaginoplasty) involving the use of tilapia skin grafts. MATERIALS ANS METHODS: A series of cases of patients treated between April 2023 and December 2024 with one of the following conditions: a non-webbed scrotum; a foreskin measuring less than 6 cm; or a history of orchiectomy that prevented the use of the scrotum as an allograft. Tilapia skin was instead chosen as the graft. RESULTS: The main intraoperative complication during vaginoplasty with foreskin inversion was lack of skin on the penis, making it unfeasible to use scrotal skin as a graft. Tilapia skin was used as an alternative, as it is a biocompatible material and an effective solution for these cases. CONCLUSIONS: Vaginoplasty using a tilapia skin xenograft is a viable surgical alternative for creating a functional neovagina with tissue resembling vaginal mucosa at a low cost with reduced morbidity.
