Abstract (English): This clinical case presents the prosthetic rehabilitation of a young permanent first molar with a mandibular hypermolar (MHM) using a milled resin overlay adhesive restoration. It shows, how an inappropriate material selection and improper handling can cause the restoration to fail, leading to further loss of tooth structure, and affecting the patient's quality of life, who reported intense pain when brushing or eating. The ill-fitting material was removed, and a restoration with cusp coverage, good sealing, and satisfactory esthetics was placed. The patient reported immediate pain relief, which generated satisfaction in her and her parents. Although this technique is not common in pediatric dentistry, its inclusion in training is recommended, since the pediatric dentists, with appropriate behavior management, can perform it effectively in a safe environment for the pediatric patient. Abstract (Spanish): Este caso clínico presenta la rehabilitación protésica de un primer molar permanente joven con hipomineralización molar incisivo (HMI) mediante una restauración adhesiva tipo overlay de resina fresada. Se evidencia cómo una elección inadecuada del material y su mal manejo pueden causar el fracaso de la restauración, mayor pérdida de estructura dental y afectar la calidad de vida del paciente, quien refirió dolor intenso al cepillarse o comer. Se retiró el material desajustado y se colocó una restauración con cobertura cuspídea, buen sellado y estética satisfactoria. La paciente reportó alivio inmediato del dolor, lo que generó satisfacción en ella y sus padres. Aunque esta técnica no es común en odontopediatría, se recomienda incluirla en su formación, ya que el odontopediatra, con un adecuado manejo de conducta, puede realizarla eficazmente en un entorno seguro para el paciente pediátrico.
