Abstract (English): Introduction: Infective endocarditis is a disease whose presentation, diagnostic techniques, and treat ment have changed over the years. The objective of this study was to analyze the results of autopsies performed on patients hospitalized for endocarditis who died as a complication of this condition. Materials and methods: Prospective registry between January 1988 and December 2023. Pathological studies were analyzed considering weight, underlying heart disease, presence of vegetations, myocardial and extra cardiac involvement. Results: A total of 296 patients were enrolled over a 35-year period. Their main characteristics were age 47±17 years, male sex (76.7%), and in-hospital mortality (83 patients) 28%. Autopsies and anatomopathological study were performed in 30 of them. The average heart weight was 543±128 g. The most common underlying heart disease was congenital (13.3%). Vegetations were present in 43.3% of cases, of which 33% were found in the aortic valve, followed by mitral valve (13%). Vegeta tions were found in more than one valve in 13% of cases, with the most prevalent being the mitral and aortic valve combination. Valve perforation was observed in 13% of cases, involving the aortic valve in 75%. Myocardial involvement due to myocarditis was described in 37%. Pulmonary congestion (53%), hepatic congestion (33%), pulmonary hypertension (30%), pulmonary thrombo embolism (20%) and acute tubular necrosis (13%) were also observed. Discussion: In a significant number of cases, vegeta tions were found, especially on the aortic valve, associ ated with myocarditis and non-cardiac complications. Abstract (Spanish): Introducción: La endocarditis infecciosa es una pa tología cuya presentación, así como las técnicas diag nósticas y el tratamiento utilizados, se han modificado con el paso de los años. El objetivo del presente estudio fue analizar los resultados de las autopsias realizadas a pacientes internados por endocarditis que fallecieron como complicación de ésta. Materiales y métodos: Registro prospectivo de las autopsias desde enero de 1988 a diciembre de 2023. Se analizaron las siguientes variables peso cardíaco, cardiopatía subyacente, presencia de vegetaciones, com promiso miocárdico y complicaciones extra cardíacas. Resultados: Se registraron 296 pacientes en 35 años. Las características principales fueron edad 47±17 años, sexo masculino 76.7%, mortalidad hospitalaria 28% (83 pacientes). Se realizó necropsia y estudio anatomopa tológico a 30 de ellos. El peso promedio del corazón fue de 543±128 g. La cardiopatía subyacente más frecuente fue la congénita (13.3%). El 43.3% presentó vegetaciones, de las cuales el 33% se encontró en la válvula aórtica, seguida por la mitral (13%). En el 13% se encontraron vegetaciones en más de una válvula, siendo la combi nación mitral y aórtica la más prevalente. La perforación valvular se observó en el 13%, comprometiendo a la vál vula aórtica en el 75%. Se describió afección miocárdica debida a miocarditis en el 37%. Se observó congestión pulmonar (53%), congestión hepática (33%), hipertensión pulmonar (30%), tromboembolismo pulmonar (20%) y necrosis tubular aguda (13%). Discusión: En un porcentaje importante de los casos se encontraron vegetaciones, especialmente en la válvu la aórtica, asociadas a complicaciones cardíacas (mio carditis) y extra cardíacas.
