Abstract (English): Introduction: Severe asthma affects approximately 3-10% of asthma patients and requires high-dose inha led corticosteroids, a second controller, and/or systemic corticosteroids for disease control. This study aimed to describe the clinical and functional changes observed in a specialized clinic for severe asthma after 12 months of follow-up. Materials and methods: A retrospective cohort study was conducted between December 2010 and January 2024. Adult patients who met the criteria for severe asth ma according to GINA 2023 and GEMA 2024 guidelines were included. Follow-up was carried out in a multidis ciplinary clinic staffed by specialists in pulmonology, otolaryngology, and allergy. Asthma control (ACT score), annual exacerbation frequency, systemic corticosteroid use, and lung function were assessed. A p-value < 0.05 was considered statistically significant. Results: A total of 72 patients were included, with a median age of 56 years; 74% were female. Systemic cor ticosteroid use decreased from 43.9% to 6% (p < 0.001). The median number of exacerbations dropped from 6 to 1 per year (p < 0.001), the ACT score increased from 14 to 22 (p < 0.001), and the average FEV1 improved from 63% to 76% of predicted value (p < 0.001). Discussion: In this retrospective cohort, significant improvements in clinical and functional variables were observed after 12 months of follow-up in patients with severe asthma. These findings support the potential usefulness of a multidisciplinary approach in the com prehensive management of severe asthma. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción: El asma grave afecta aproximadamente al 3-10% de los pacientes asmáticos y requiere corti costeroides inhalados en altas dosis, un segundo con trolador y/o corticosteroides sistémicos para lograr su control. El presente estudio tuvo como objetivo describir los cambios clínicos y funcionales observados en un consultorio especializado en asma grave tras 12 meses de seguimiento. Materiales y métodos: Se realizó un estudio de co horte retrospectivo entre diciembre de 2010 y enero de 2024. Se incluyeron pacientes adultos con diagnóstico de asma grave, según los criterios de las guías GINA 2023 y GEMA 2024. El seguimiento se llevó a cabo en un consultorio interdisciplinario integrado por especialis tas en neumonología, otorrinolaringología y alergia. Se evaluaron el control del asma mediante el cuestionario ACT, la frecuencia anual de exacerbaciones, el uso de corticosteroides sistémicos y la función pulmonar. Se consideró estadísticamente significativa una p < 0.05. Resultados: Se incluyeron 72 pacientes, con una mediana de edad de 56 años, de los cuales el 74% eran mujeres. El uso de corticosteroides sistémicos se redujo del 43,9% al 6% (p < 0,001). Las exacerbaciones dismi nuyeron de 6 a 1 por año (p < 0.001), el puntaje ACT aumentó de 14 a 22 (p < 0.001) y el FEV1 promedio pasó del 63% al 76% del valor predicho (p < 0.001). Discusión: En esta cohorte retrospectiva se observó que, tras 12 meses de seguimiento, los pacientes con asma grave presentaron una mejora significativa en variables clínicas y funcionales.
