Abstract (English): BACKGROUND: Myelomeningocele and its treatment can lead to various complications for the mother and foetus. OBJECTIVE: To compare the incidence of complications in mothers and fetuses undergoing open prenatal surgery or fetoscopy for a myelomeningocele diagnosis. Additionally, we aimed to evaluate the neurological and functional outcomes of newborns under the aforementioned conditions. METHOD: A systematic review was conducted using the DeCS and MeSH thesauri to search the PubMed, Web of Science and Cochrane databases. The PICO question was used to determine the inclusion and exclusion criteria, and the PRISMA diagram and JBI checklist were used for article selection. Evidence levels (Oxford) were also considered. RESULTS: Seven retrospective cohort studies were included, which overall revealed fewer complications for the mother and foetus with the fetoscopic approach. With the exception of surgery duration and retinopathy incidence, which favoured open surgery, no differences were found in newborn neurological development. CONCLUSIONS: Fetoscopy is associated with fewer complications for the mother and foetus than open surgery. However, these interventions do not appear to affect the neurodevelopment of the newborn. Abstract (Spanish): ANTECEDENTES: El mielomeningocele y su intervención pueden dar lugar a diversas complicaciones en la madre y en el feto. OBJETIVO: Comparar las complicaciones en la madre y en el feto entre la cirugía prenatal abierta y la fetoscopia, en fetos con diagnóstico de mielomeningocele. Además, evaluar el desenlace neurológico y funcional del neonato en las condiciones descritas. MÉTODO: Revisión sistemática efectuada con el uso de tesauros DeCS y MeSH para la búsqueda en las bases de datos de PubMed, Web of Science y Cochrane. Los criterios de selección de artículos siguieron la pregunta PICO, con criterios de inclusión y exclusión, el diagrama PRISMA, la lista de verificación jbi.global y los niveles de evidencia (Oxford). RESULTADOS: Se incluyeron siete estudios de cohortes retrospectivas que, de manera global, revelaron menores complicaciones para la madre y el feto con la vía fetoscópica. A excepción de la duración de la cirugía y la incidencia de retinopatía, que apuntaba a favor de la cirugía abierta, no se encontraron diferencias en el desarrollo neurológico del neonato. CONCLUSIONES: La fetoscopia se asocia con menos complicaciones para la madre y para el feto, en comparación con la cirugía abierta.
