Abstract (English): Objective: To compare efficiency, time, cost, and adherence of environmental cleaning protocols in dentistry between the United States (USA) and Brazil. Methods: Systematic review (PRISMA 2020) in PubMed, SciELO, LILACS, EPA, ANVISA, CDC, and CFO (2018-2025). In vitro, clinical studies, and guidelines were included. Independent screening, quality assessed by AMSTAR-2/ROBINS-I. Results: 428 records identified; 28 studies included (15 USA, 10 Brazil). USA: 6.4 min, 99.1% bacterial reduction, US$0.13/m², 94% adherence, 73% HEPA/UV use. Brazil: 14.8 min, 93.8% reduction, R$0.10/m², 71% adherence, 20% technology use. Survey confirmed: 100% USA use spray <10 min; 80% Brazil use manual cleaning >12 min. Conclusion: U.S. protocols are faster and more effective but costly. Métodos: Revisión sistemática (PRISMA 2020) en PubMed, SciELO, LILACS, EPA, ANVISA, CDC y CFO (2018-2025). Se incluyeron estudios in vitro, clínicos y basados en guías de práctica clínica. Selección independiente y evaluación de la calidad mediante AMSTAR-2/ROBINS-I. Resultados: Se identificaron 428 registros; se incluyeron 28 estudios (15 de EE. UU. y 10 de Brasil). EE. UU.: tiempo 6,4 min, reducción bacteriana 99,1 %, costo US$0,13/m², adherencia 94 %, 73 % con HEPA/UV. Brasil: tiempo 14,8 min, reducción 93,8 %, costo R$0,10/m², adherencia 71 %, 20 % con tecnología. La encuesta confirmó que el 100 % de los profesionales en EE. UU. utilizan aerosoles en menos de 10 minutos. En Brasil, el 80 % de los casos requieren limpieza manual durante más de 12 minutos. Conclusión: Los protocolos estadounidenses son más rápidos y eficaces, pero costosos. Los protocolos brasileños son accesibles, pero requieren mucho tiempo y presentan baja adherencia.
