Abstract (English): Objective: To characterize the indications and findings of colonoscopies performed in individuals under 45 to evaluate diagnostic yield. Materials and Methods: Analytical cross-sectional study of 1000 patients ≤ 45 years who underwent colonoscopy (2022–2024, Clínica INDISA). Inadequate preparations were excluded. Patients with and without significant findings (polyps > 1 cm, high grade dysplasia, colorectal cancer (CRC), etc.) were compared using logistic regression. Results: A total of 5.9% presented significant findings, the most frequent being polyps ≥ 1 cm (50.8%) and ≥ 3 adenomas (23.7%). Colorectal cancer was diagnosed in 0.5% of cases. Bleeding was the most common indication. The group with significant findings were older and had a higher proportion of males (p = 0.0203 and p = 0.0263, respectively). Logistic regression showed a significant association with a history of polyps (OR: 6.62; p < 0.001). Discussion: Colonoscopy in young adults shows low diagnostic yield, especially in nonspecific indications. These findings support a more selective approach to patient referral. Conclusion: Colonoscopy is safe in young adults but its utility should be optimized by prioritizing high-risk cases. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Objetivo: Caracterizar las indicaciones y hallazgos de las colonoscopías realizadas en menores de 45 años para evaluar su rendimiento diagnóstico. Materiales y Métodos: Estudio transversal analítico de 1.000 pacientes de ≤ 45 años sometidos a colonoscopía entre 2022 y 2024 en Clínica INDISA. Se excluyeron estudios con preparación inadecuada. Se compararon grupos con y sin hallazgos significativos (pólipos > 1 cm, displasia de alto grado, cáncer colorrectal (CCR), etc.) mediante regresión logística. Resultados: El 5,9% presentó hallazgos significativos, siendo los más frecuentes pólipos ≥ 1 cm (50,8%) y ≥ 3 adenomas (23,7%). El 0,5% tuvo diagnóstico de CCR. El sangrado fue la indicación más común. El grupo con lesiones tuvo mayor edad y sexo masculino en mayor proporción (p = 0,0203 y p = 0,0263). La regresión logística mostró asociación significativa con antecedente de pólipos (OR: 6,62; p < 0,001). Discusión: La colonoscopía en jóvenes tiene bajo rendimiento diagnóstico, especialmente en indicaciones inespecíficas. Los hallazgos refuerzan la necesidad de una mejor selección de pacientes y uso racional del examen.
