Abstract (English): Functional diarrhea is the leading cause of chronic diarrhea in apparently healthy children under five years of age and is part of the disorders of gut-brain interaction. Its diagnosis is based on the Rome IV Criteria, which describe daily, painless, large, unformed bowel movements lasting more than four weeks. Its pathophysiology includes luminal factors, such as excessive intake of osmotically active sugars, and alterations in intestinal motility. Although its prevalence is low (1.0-3.0%), it generates great concern among caregivers, leading to an overuse of unnecessary diagnostic tests. Management focuses on dietary guidance and family reassurance, without requiring specific medical treatments. Despite its frequency as a cause of pediatric consultation, functional diarrhea remains understudied in Latin America, underscoring the need for more extensive research into its clinical management. Abstract (Spanish): La diarrea funcional es la principal causa de diarrea crónica en niños aparentemente sanos menores de cinco años y forma parte de los Trastornos del Eje Cerebro-Intestino. Su diagnóstico se basa en los Criterios de Roma IV, que describen evacuaciones diarias, indoloras, grandes y no formadas, con una duración superior a cuatro semanas. La fisiopatología incluye factores luminales, como la ingesta excesiva de azúcares osmóticamente activos, y alteraciones en la motilidad intestinal. Aunque su prevalencia es baja (1,0-3,0%), genera gran preocupación en los cuidadores, lo que conlleva un uso excesivo de pruebas diagnósticas innecesarias. El manejo se enfoca en la orientación dietaria y la tranquilidad a la familia, sin requerir tratamientos médicos específicos. A pesar de su frecuencia como causa de consulta pediátrica, la diarrea funcional sigue siendo poco estudiada en América Latina, lo que subraya la necesidad de investigaciones más amplias sobre su abordaje clínico.
