Abstract (English): Pediatric urolithiasis is an increasingly common condition, with multiple factors involved in its etiology. Recently, a potential association has been suggested between the use of antibiotics, particularly cephalosporins, and kidney stone formation, mediated by alterations in the gut and urinary microbiota. Objective: To conduct a narrative review on the relationship between antibiotics and pediatric urolithiasis, identifying proposed mechanisms and associated clinical risks. Method: A narrative review of the literature was performed using PubMed, Scopus, and Google Scholar. Original studies published between 2004 and 2025 were selected, focusing on the pediatric population and addressing the link between antibiotic use and stone formation. Given the limited evidence available for certain antibiotic classes, relevant case reports were also included. Results: Potential mechanisms identified included intestinal and urinary dysbiosis, altered oxalate metabolism, changes in urinary pH, and urinary supersaturation. Most studies focused on ceftriaxone, limiting the generalizability of the findings. Conclusion: The use of antibiotics in children should be carefully evaluated, especially in those with predisposing factors for lithiasis. Further research is needed to better understand this relationship and develop effective preventive strategies. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): La urolitiasis pediátrica es una entidad en aumento, con múltiples factores implicados en su etiología. Recientemente, se ha planteado una posible asociación entre el uso de antibióticos, especialmente cefalosporinas, y la formación de cálculos renales, mediada por la alteración de la microbiota. Objetivo: Realizar una revisión narrativa sobre la relación entre los antibióticos y la urolitiasis pediátrica, identificando mecanismos propuestos y riesgos clínicos asociados. Método: Revisión narrativa de literatura mediante búsqueda en PubMed, Scopus y Google Scholar. Se seleccionaron estudios originales entre 2004 y 2025, centrados en población pediátrica y que abordaran el vínculo entre el uso de antibióticos y la formación de cálculos. Dada la escasez de evidencia en ciertas clases antibacterianas, se incluyeron también reportes de casos clínicos relevantes. Resultados: Se identificaron mecanismos potenciales como disbiosis intestinal y urinaria, alteración del metabolismo del oxalato, cambios en el pH urinario y sobresaturación urinaria. La mayoría de estudios se centraron en ceftriaxona, limitando la generalización. Conclusión: El uso de antibióticos debe ser cuidadosamente valorado en niños, especialmente en aquellos con factores predisponentes para litiasis.
