Abstract (English): Community-acquired pneumonia (CAP) is a significant cause of morbidity and mortality in adolescents. This developmental stage involves physiological changes that affect lung structure and the immune response. Obesity, whose prevalence is steadily increasing in this population, adds clinical complexity, as it is considered a chronic, low-grade inflammatory condition that pre-exists the acute illness associated with respiratory dysfunction and metabolic alterations that can negatively impact the course of the disease. This review examines the impact of adolescence, obesity, and their combination on the clinical presentation, progression, and treatment of CAP. Available evidence suggests that obese adolescents experience more frequent respiratory symptoms, lower oxygen saturation, recurrent infections, increased need for respiratory support, intensive care unit admission, longer hospital stays, and higher associated costs. Furthermore, pharmacokinetic alterations specific to obese adolescents, such as changes in distribution volume, hepatic metabolism, and renal clearance, may lead to antimicrobial underdosing and increase the risk of treatment failure. Both obesity and adolescence significantly modify the clinical course of CAP, increasing its severity and complicating its management. A point of discussion is that most studies group together broad pediatric age ranges without analyzing the adolescent subgroup separately, limiting our understanding of their specific clinical characteristics. This review evaluates the available information to determine whether the combination of obesity and adolescence could define a distinctive clinical profile, characterized by specific physiological and metabolic features that warrant further investigation in studies specifically targeting this age group. Abstract (Spanish): La neumonía adquirida en la comunidad (NAC) constituye una causa significativa de morbimortalidad en adolescentes. Esta etapa presenta cambios fisiológicos que modifican la estructura pulmonar y la respuesta inmunitaria. La obesidad, cuya prevalencia va en aumento de forma sostenida en este grupo, añade complejidad clínica al considerarse un estado inflamatorio crónico de bajo grado preexistente a la enfermedad aguda asociado a disfunción ventilatoria y alteraciones metabólicas que pueden influir negativamente en la evolución de la enfermedad. Esta revisión examina el impacto de la adolescencia, la obesidad y su combinación en la presentación clínica, progresión y tratamiento de la NAC. La evidencia disponible sugiere que los adolescentes con obesidad presentan mayor frecuencia de síntomas respiratorios, desaturación, recurrencia de cuadros infecciosos, necesidad de soporte ventilatorio, ingreso a cuidados intensivos, estancia hospitalaria prolongada y aumento en los costos asociados. Además, las alteraciones farmacocinéticas propias de los adolescentes con obesidad, como cambios en el volumen de distribución, metabolismo hepático y aclaramiento renal, pueden favorecer la subdosificación de antimicrobianos y aumentar el riesgo de fracaso terapéutico. La obesidad y la adolescencia modifican de forma significativa el curso clínico de la NAC, incrementando su gravedad y dificultando su manejo. Un aspecto en discusión es que la mayoría de los estudios agrupa amplias franjas etarias pediátricas sin analizar de forma diferenciada al subgrupo adolescente, lo que limita la comprensión de sus particularidades clínicas. En esta revisión se evalúa la información disponible para evaluar si la confluencia de obesidad y adolescencia podrían configurar un perfil clínico distintivo, caracterizado por particularidades fisiológicas y metabólicas que amerita ser explorado en futuras investigaciones específicamente dirigidas a este grupo etario.
