Abstract (English): Background: Kidney Failure (KF) leads to loss of productive years, premature mortality, and substantial social burden due to work-related disability. Objective: To characterize temporary and permanent work disability transfers attributable to KF among affiliates of the Mexican Social Security Institute (IMSS). Material and methods: Patterns of use were analyzed for: (a) disability pensions at the national and regional levels, and (b) temporary work disability (TWD) at the healthcare facility level. All monetary values are expressed in 2024 Mexican pesos (MXN). Results: A decreasing trend in newly granted KF-related disability pensions was identified over time. In 2024, 690 new pensions were reported, 24% of which were for women, with the highest proportions in Jalisco (26%) and the State of Mexico (14%). Temporary work disability was observed in 24% of insured individuals who ultimately returned to work, lasting on average 70 days (SD 71) and costing MXN 11,534 (SD 12,347). Individuals on automated peritoneal dialysis had shorter TWD durations (p < 0.05). Among new pension recipients, 88% had a preceding TWD episode lasting an average of 95 days (95% CI: 82-112), equivalent to MXN 16,102 (95% CI: 12,866-18,339). The estimated average annual pension, weighted by salary range, was MXN 74,218. On average, 17.4 productive years (95% CI: 16-19) and 27.3 years lived with disability were lost per case. Conclusions: Standardizing IMSS procedures aimed at assessing and mitigating work disability could substantially reduce the burden of KF for patients, their families, and society. Abstract (Spanish): Introducción: la insuficiencia renal crónica (IRC) genera años productivos perdidos, muerte prematura y elevada carga social derivada de la discapacidad laboral. Objetivo: caracterizar las transferencias por incapacidad laboral temporal y permanente debidas a IRC en afiliados al Instituto Mexicano del Seguro Social (IMSS). Material y métodos: se analizaron los patrones de uso de: a) pensiones por invalidez a nivel nacional y delegacional, y b) incapacidad temporal para el trabajo (ITT) a nivel de unidad médica. Los valores se expresan en pesos mexicanos (MXN) de 2024. Resultados: se identificó una reducción en el número de pensiones iniciales por IRC a lo largo del tiempo. En 2024 se emitieron 690 pensiones, siendo el 23% para las mujeres; las delegaciones con más pensiones fueron Jalisco (26%) y Estado de México (14%). El 24% de los afiliados que recibieron ITT concluyó en reincorporación laboral; la duración promedio de la ITT fue de 70 días (DE 71), con un monto promedio de $11,534 (DE $12,347). En personas con diálisis peritoneal automatizada se observó una menor duración de ITT (p < 0.05). El 88% de las pensiones iniciales estuvo precedido por ITT, con una duración promedio de 95 días (IC95%: 82 - 112), equivalente a $16,102 (IC95%: $12,866 - $18,339). El monto estimado de una pensión anual ponderada por rangos salariales fue de $74,218. Se estimó además una pérdida de 17.4 años (IC95%: 16 - 19) de vida productiva y 27.3 años de vida vividos con discapacidad. Conclusiones: la estandarización de procedimientos dirigidos a evaluar y reducir la discapacidad laboral en el IMSS contribuye a disminuir la carga de la enfermedad para el paciente, su familia y la sociedad.
