Abstract (English): Introduction: Ocrelizumab has been used in patients with multiple sclerosis as a disease-modifying therapy. Studies have shown that it is beneficial in slowing relapses. Its use was introduced in Cuba, but no research had been conducted to determine the response to this therapy in our setting. Objective: to determine the impact of treatment with Ocrelizumab on relevant variables corresponding to patients with relapsing-remitting multiple sclerosis treated in a Cuban tertiary care center over four years, using a mixed approach of related measures and combined effect analysis. Methods: observational, descriptive, prospective study in patients of both sexes, 15 treated with ocrelizumab for one or more years, from march 2019 to march 2024 and a comparative group of 15 patients treated with rebif. The study variables were: disability measured with the EDSS scale, clinical and radiological relapses, annual hospital admission and NEDA. Results: Ocrelizumab significantly decreased radiological and clinical progression of the disease, as well as the number of hospital admissions, with a difference of no evidence of disease activity before and after ocrelizumab of 86.67% in the first year, 75.00% in the second year and 77.78% in the third year (p: 0.0002, P:0.0039 and p: 0.0156, respectively). In the forest plot analysis there is an effect in favor of Ovrelizumab in: reduction in the number of admissions, number of clinical flares, number of lesions in MRI and in no evidence of disease activity compared to Rebif. Conclusions: Treatment with ocrelizumab showed a favorable impact in: reduction in the number of admissions, number of clinical flares, radiological lesions in magnetic resonance imaging and no evidence of disease activity over the four years evaluated and in comparison, with the use of Rebif in patients with multiple sclerosis treated in a Cuban tertiary care center. Abstract (Spanish): Introducción: El ocrelizumab ha sido utilizado en pacientes con esclerosis múltiple como terapia modificadora de la enfermedad, los estudios realizados han mostrado beneficio para frenar las recaídas. En Cuba se introdujo su uso, pero no habían sido realizadas investigaciones que permitieran conocer la respuesta a esta terapéutica en nuestro medio. Objetivo: determinar el impacto del tratamiento con ocrelizumab en variables relevantes correspondientes a pacientes con esclerosis múltiple remitente recurrente atendidos en un centro cubano de atención terciaria a lo largo de cuatro años, aplicando un enfoque mixto de medidas relacionadas y análisis de efecto combinado. Métodos: estudio observacional, descriptivo, prospectivo en pacientes de ambos sexos, 15 tratados con ocrelizumab, por uno o mas años, desde marzo del 2019 a marzo 2024 y un grupo de 15 pacientes comparativo tratados con Rebif. Las variables de estudio fueron: la discapacidad medida con la escala de EDSS, recaídas clínicas y radiológicas, ingresos hospitalarios anuales y no evidencia de actividad de la enfermedad. Resultados: El ocrelizumab disminuyó significativamente la progresión radiológica y clínica de la enfermedad, así como el número de ingresos hospitalarios, observándose una diferencia de no evidencia de actividad de la enfermedad antes y despés de ocrelizumab de un 86,67 % al primer año, 75,00 % en el segundo y 77, 78 % al tercer año (p: 0.0002, P:0.0039 y p: 0.0156, respectivamente). En el análisis de los forest plot existe efecto a favor del ocrelizumab en: reducción del número de ingresos, número de brotes clínicos, número de lesiones en resonancia magnética y en no evidencia de actividad de la enfermedad en comparación con el Rebif. Conclusiones: El tratamiento con ocrelizumab mostró un impacto favorable en: reducción del número de ingresos, número de brotes clínicos, lesiones radiológicas en resonancia magnética y no evidencia de actividad de la enfermedad a lo largo de los cuatro años evaluados y en comparación con el uso de Rebif en pacientes con esclerosis multiple tratados en un centro cubano de atención terciaria.
