Abstract (English): Objective: to determine the prevalence of difficulty in lower impacted third molar surgery, in a panoramic radiographic study, in patients at an imaging diagnostic center, from January to July 2022, Lima, Peru. Method: descriptive and retrospective study in which a panoramic radiographic study was performed on the files of patients treated at an imaging diagnostic center from January to July 2022. The level of complexity prior to surgery was analyzed using the Peñarrocha index. Results: A total of 215 impacted lower third molars were evaluated, originating from 141 patients, 51.8% of whom were female. The most prevalent age group was 20 to 29 years old, accounting for 30.5%. The most common preoperative complexity level was moderate, accounting for 77.2%, followed by low difficulty, accounting for 16.3%. Conclusions: Lower third molars were found to be more prevalent at a medium level of pre-surgical complexity. Abstract (Spanish): Objetivos: determinar la prevalencia de la dificultad de la cirugía de terceros molares inferiores impactados, en un estudio radiográfico panorámico, en pacientes de un centro de diagnóstico por imágenes, de enero a julio del 2022, Lima - Perú. Método: estudio descriptivo y retrospectivo donde se realizó un estudio radiográfico panorámico del archivo de pacientes tratados en un centro de diagnóstico por imágenes de enero a julio 2022, se analizó el nivel de complejidad antes de la cirugía mediante el uso del índice de Peñarrocha. Resultados: se evaluaron 215 terceros molares inferiores impactados, que provenían de 141 pacientes, perteneciendo el 51,8% al sexo femenino y el grupo etario más prevalente fue el que alcanzaba de 20 a 29 años con 30,5%. El nivel de complejidad preoperatoria más observada fue nivel medio, alcanzando el 77,2 %, seguidamente por el nivel de escasa dificultad que alcanzó un 16,3%. Conclusiones: las terceras molares inferiores se hallaron con prevalencia mayor en un nivel de complejidad prequirúrgico de grado medio. [ABSTRACT FROM AUTHOR]
