Abstract (English): Full-arch rehabilitation of edentulous maxillary patients represents a clinical challenge, mainly due to bone resorption, anatomical limitations, and the functional and psychosocial impact caused by tooth loss. The All-on-Four protocol emerges as a predictable therapeutic approach that enables fixed rehabilitation with immediate loading. This study aimed to review scientific evidence regarding the applicability, clinical performance, functional benefits, and limitations of the All-on-Four concept in the maxilla, with emphasis on predictability, longevity, and impact on quality of life. The literature demonstrates high success and prosthetic survival rates, in addition to significant improvements in mastication, phonetics, and patient satisfaction, when proper planning is combined with accurate case selection and skilled clinical execution. Evidence indicates that the protocol enables less invasive treatment, shorter clinical time, and faster recovery, without compromising stability or esthetic outcomes. Therefore, the All-on-Four technique is a modern, effective, and scientifically supported alternative for full-arch maxillary rehabilitation, provided that reverse planning, precise diagnosis, and surgical-prosthetic expertise are properly applied. Abstract (Portuguese): A reabilitação total de pacientes edêntulos na maxila representa um desafio clínico, principalmente em função da reabsorção óssea, limitações anatômicas e impacto funcional e psicossocial da perda dentária. Porém o protocolo All-on-Four surge como uma abordagem terapêutica previsível que permite reabilitação fixa com carga imediata. O presente estudo teve como objetivo revisar evidências científicas sobre a aplicabilidade, desempenho clínico, benefícios funcionais e limitações do All-on-Four na maxila, com ênfase na previsibilidade, longevidade e impacto na qualidade de vida. A literatura demonstra elevadas taxas de sucesso e sobrevida protética, além de melhora significativa na mastigação, fonética e satisfação do paciente, quando o planejamento é realizado de forma criteriosa, associado à execução técnica adequada e seleção correta dos casos. Foi observado que a utilização do protocolo favorece tratamentos menos invasivos, com menor tempo de tratamento e recuperação mais rápida, sem comprometer a estabilidade e a reabilitação estética. Portanto, a técnica All-on-Four é uma alternativa moderna, eficiente e clinicamente consolidada para a reabilitação total da maxila, desde que sustentada por planejamento reverso, diagnóstico preciso e domínio técnico operacional.
