Abstract (English): Prosthetic rehabilitation of severely atrophic maxillae remains a significant clinical challenge, particularly when the available bone volume is insufficient for the placement of conventional implants. In this scenario, zygomatic implants emerge as a viable and predictable therapeutic alternative, allowing the installation of fixed protocol-type prostheses with high success rates. This article presents a clinical case report of a 65-year-old female patient with maxillary edentulism and severe bone resorption, who underwent the All-on-Four technique associated with the placement of two zygomatic implants and two anterior conventional implants. Immediate loading was performed using a prefabricated fixed provisional prosthesis following three-dimensional planning and conservative prosthetic molding. The procedure achieved excellent primary stability, absence of significant complications, and favorable tissue adaptation, resulting in satisfactory functional and aesthetic outco-mes. This case reinforces the importance of the technique's mastery by dental surgeons as a safe and effective option for the rehabilitation of patients with advanced maxillary atrophy, highlighting the need for integration between surgical and prosthetic planning to ensure predictability, comfort, and improved quality of life for the patient. Abstract (Portuguese): A reabilitação protética de maxilas severamente atróficas representa um desafio clínico significativo, especialmente quando o volume ósseo disponível inviabiliza a instalação de implantes convencionais. Nesse contexto, os implantes zigomáticos surgem como uma alternativa terapêutica viável e previsível, permitindo a instalação de próteses fixas do tipo protocolo com alta taxa de sucesso. O presente trabalho apresenta um relato de caso clínico de uma paciente de 65 anos, com edentulismo maxilar e importante reabsorção óssea, submetida à técnica All-on-Four associada à instalação de dois implantes zigomáticos e dois implantes convencionais anteriores. Foi realizada carga imediata com prótese provisória fixada aos implantes, após planejamento tridimensional e moldagem protética ultraconservadora. O procedimento apresentou excelente estabilidade primária, ausência de complicações significativas e adequada adaptação tecidual, com resultados funcionais e estéticos satisfatórios. O caso reforça a importância do domínio da técnica pelo cirurgião-dentista como opção segura e eficaz para reabilitação de pacientes com atrofia maxilar avançada, evidenciando a necessidade de integração entre o planejamento cirúrgico e o protético, a fim de obter previsibilidade, conforto e qualidade de vida ao paciente.
