Abstract (English): Objective: To describe the conservative clinical approach to an inmature permanent anterior tooth with traumatic pulp exposure treated total pulpotomy using a mineral-trioxide-aggregate-derived bioceramic cement (Neoputty), in order to maintain pulpal vitality and allow continued root development. Case report: A 9-year-9-month-old male patient presented 72 hours after suffering dental trauma. The patient's mother reported sensitivity to cold and pain during mastication. Intraoral examination revealed a complicated crown fracture with pulp exposure and tenderness to percussion affecting the upper left permanent central incisor (tooth 21). Radiographic assessment revealed the crown fracture with pulpal involvement, periodontal ligament widening and an open apex. A total pulpotomy was performed using a bioactive bioceramic material (NeoPutty), followed by clinical and radiographic follow-ups at 2, 5, and 8 months. The treated tooth remained asymptomatic, exhibited no discoloration, and demonstrated appropriate apical development. Conclusion: Total pulpotomy proved to be an effective conservative treatment option for a young permanent tooth with traumatic pulp exposure, preserving pulpal vitality and promoting physiological apexogenesis. Abstract (Spanish): Objetivo: Describir el abordaje clínico conservador de un diente anterior permanente joven con exposición pulpar traumática mediante pulpotomía con un cemento biocerámico derivado del agregado trióxido mineral (NeoPutty) para mantener la vitalidad pulpar y permitir el desarrollo radicular. Reporte de Caso: Paciente de sexo masculino de 9 años y 9 meses, acudió a consulta 72 horas después de sufrir un traumatismo dental. La madre refiere que presenta sensibilidad al frío y dolor al momento de comer. Al examen clínico intraoral se observa fractura coronaria con exposición pulpar y sensibilidad a la percusión en el incisivo central superior izquierdo (pieza 21). En el examen radiográfico se observó fractura de la corona con compromiso pulpar, ensanchamiento de ligamento periodontal y presencia de ápice abierto a nivel radicular. Se realizó una pulpotomía total utilizando cemento biocerámico biactivo (NeoPutty), y controles clínico-radiográficos a los 2, 5 y 8 meses. En el último control la pieza 21 se presentó asintomática, sin cambios de coloración y con un desarrollo apical adecuado. Conclusiones: La pulpotomia realizada con un cemento biocerámico bioactivo (Neoputty) resultó ser una intervención conservadora con un buen desempeño clínico en un diente permanente joven con exposición pulpar debido a un trauma dental, logrando preservar la vitalidad pulpar y estimular el cierre del ápice.
