Abstract (English): Objective: to determine the diagnostic capacity of serum albumin levels for identifying pediatric patients with complicated appendicitis. Material and methods: a diagnostic test study was conducted on 210 patients aged 3 to 17 years with a surgical diagnosis of acute appendicitis from three hospitals in Puebla, Mexico, between 2023 and 2024. Clinical and laboratory data were compared between patients with complicated appendicitis (n = 125) and those with simple appendicitis (n = 85). Results: albumin levels were lower in patients with complicated appendicitis (3.87 g/dL) than in those with simple appendicitis (4.35 g/dL), p < 0.001. The cutoff point of 3.8 g/dL showed a sensitivity of 60%, specificity of 95%, positive predictive value of 80%, and negative predictive value of 49%. Conclusions: serum albumin levels have limited utility in identifying patients with complicated appendicitis. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Objetivo: determinar la capacidad diagnóstica de los niveles de albúmina sérica para la identificación de pacientes pediátricos con apendicitis complicada. Material y métodos: estudio de prueba de diagnóstico, realizado en 210 pacientes de entre 3 y 17 años, con diagnóstico quirúrgico de apendicitis aguda, de tres hospitales en Puebla de 2023 a 2024. Se compararon los datos clínicos y de laboratorio entre pacientes con apendicitis complicada (n = 125) con aquellos de apendicitis simple (n = 85). Resultados: los niveles de albúmina fueron menores en pacientes con apendicitis complicada (3.87 g/dL) que aquellos con apendicitis simple (4.35 g/dL), p < 0.001. El punto de corte de 3.8 g/dL mostró una sensibilidad del 60%, especificidad del 95%, valor predictivo positivo del 80% y valor predictivo negativo del 49%. Conclusiones: los niveles séricos de albúmina para la identificación de pacientes con apendicitis complicada tienen una utilidad limitada. [ABSTRACT FROM AUTHOR]
