Abstract (English): This document is an updated consensus developed jointly by the Argentine Society of Infectious Diseases and the Argentine Society of Intensive Care Medicine, revising the original 2018 recommendations on the prevention, diagnosis, and treatment of Central Line- Associated Bloodstream Infections (CLABSI). The meth odology employed was a qualitative review of literature published from 2018 to 2024, supplemented by expert opinion and local epidemiological data. Among the main changes are the differentiation between clinical and epidemiological definitions, the proposal regarding the utility of catheter tip culture in diagnosis, and, concerning management, the provision of scenarios for CVC preservation, shortened treatment durations, the development of lock therapy, and the inclusion of complication management. The objective of this update is to revise and strength en current recommendations for the prevention, di agnosis, and management of CLABSI. It also provides practical tools aimed at optimizing each of these as pects clinical and microbiological diagnostic strategies, preventive measures including care bundles, and thera peutic approaches. With respect to treatment, the docu ment outlines both empirical and targeted antimicrobial regimens, which should be tailored to local resistance patterns. It also includes guidance on the appropriate dosing and administration of antimicrobials in critically ill patients with CLABSI. Furthermore, the document presents a diagnostic and treatment algorithm to support clinical decision-making and standardize care. This multidisciplinary initiative underscores the importance of a comprehensive ap proach to CLABSI, with the ultimate goal of preventing infections and optimizing their management in order to reduce morbidity, mortality, length of hospital stay, healthcare costs, and the emergence of antimicrobial resistance. Abstract (Spanish): Representantes de la Sociedad Argentina de Infec tología y la Sociedad Argentina de Terapia Intensiva se reunieron para trabajar en la actualización de las reco mendaciones elaboradas en 2018 sobre el diagnóstico, tratamiento y prevención de las bacteriemias asociadas a catéteres venosos centrales (BAC). La metodología utilizada fue una revisión cualitativa de la literatura publicada desde 2018 a 2024, complementada con la opinión de expertos y datos locales. Entre los principales cambios, se destacan la diferen ciación de las definiciones clínicas y epidemiológicas, el planteamiento de la utilidad del cultivo de punta de catéter en el diagnóstico y, con respecto al manejo, la provisión de escenarios para la conservación del CVC, duraciones acortadas de tratamiento, el desarrollo de la terapia de bloqueo y la incorporación del manejo de las complicaciones. Para optimizar estos aspectos, el documento ofre ce herramientas prácticas como paquetes de medidas para la prevención, así como la descripción de posibles esquemas antibióticos empíricos y dirigidos para el tra tamiento, que deberán ser adaptados a la epidemiología local. También se aborda la posología y administración de antimicrobianos en pacientes críticos con BAC. Todas las recomendaciones se encuentran resumidas en un algoritmo de diagnóstico y tratamiento para facilitar su aplicación. Este trabajo conjunto multidisciplinario, resalta la necesidad de abordar esta problemática en forma inte grada. El objetivo final es una causa en común, prevenir las BAC, optimizar su diagnóstico y tratamiento para disminuir la morbimortalidad, los días de internación, los costos y la resistencia antimicrobiana que se asocian a las BAC.
