| Abstract (English): | The extended hemicoronal approach (EHCA) provides broad access to all three facial thirds, which is useful in complex panfacial fractures. The case of a polytrauma male with right panfacial fracture, after sustaining a high-energy motorcycle accident, is presented. Clinically, he was hemodynamically stable, and on examination, hemifacial volume increase, edema, periorbital ecchymosis, conjunctival chemosis, restriction of mandibular opening, and anterior open bite were noted. CT scan showed multiple fractures in the three facial thirds. EHCA was performed, supplemented with transconjunctival and upper vestibular accesses, which allowed accurate anatomical reduction and stable fixation by means of osteosynthesis plates. The patient recovered without significant functional or aesthetic sequelae. This case demonstrates that the EHCA is effective and versatile for the comprehensive management of complex panfacial fractures. [ABSTRACT FROM AUTHOR] |
| Abstract (Spanish): | El abordaje hemicoronal extendido (AHCE) proporciona un amplio acceso a los tres tercios faciales, lo cual resulta útil en fracturas panfaciales complejas. Se presenta el caso de un paciente politraumatizado con fractura panfacial derecha, tras sufrir un accidente de alta energía en motocicleta. En la revisión clínica, se mostró hemodinámicamente estable y al examen destacaban el aumento de volumen hemifacial, edema, equimosis periorbitaria, quemosis conjuntival, restricción de apertura mandibular y mordida abierta anterior. La tomografía mostró múltiples fracturas en los tres tercios faciales. Se realizó AHCE, complementado con accesos transconjuntival y vestibular superior, lo que permitió realizar la reducción anatómica precisa y fijación estable mediante placas de osteosíntesis. La evolución fue favorable, sin secuelas funcionales ni estéticas significativas. Este caso demuestra que el AHCE es eficaz y versátil para el manejo integral de fracturas panfaciales complejas.
