Abstract (English): BACKGROUND: Thyroid gland surgery is one of the most common surgical procedures worldwide; the most frequent thyroid diseases requiring surgery are multinodular goiter and malignant neoplasms. Despite being a routine procedure, postoperative complications can significantly affect patients' quality of life and even be life- threatening. Therefore, advanced surgical techniques and equipment have been developed that allow for the identification of key anatomical structures and reduce the risk of complications, such as the robotic microscope or exoscope. CLINICAL CASE: A 48-year-old male patient with giant multinodular goiter in whom a right hemithyroidectomy assisted by the RoboticScope robotic microscope was performed to facilitate the identification of key anatomical structures. The surgery was performed at the Dr. Manuel Gea Gonzalez General Hospital in June 2024. The robotic microscope facilitated the precise identification of the recurrent laryngeal nerve, inferior parathyroid gland, and inferior thyroid artery, allowing accurate and safe dissection. The results and quality of exoscope use were evaluated using the exoscope quality assessment tool. CONCLUSIONS: The use of the robotic microscope is promising for neck surgery, enhancing the identification of key anatomical structures, reducing complications, and offering ergonomic benefits. Although it has limitations such as a learning curve and cost, its advantages outweigh the drawbacks. Further research of its application in complex surgeries with the mentioned tool is recommended. Abstract (Spanish): ANTECEDENTES: La cirugía de la glándula tiroides es una de las más frecuentes en todo el mundo; las enfermedades tiroideas quirúrgicas más comunes son el bocio multinodular y las neoplasias malignas. A pesar de ser un procedimiento habitual, las complicaciones posquirúrgicas pueden afectar significativamente la calidad de vida de los pacientes e, incluso, poner en riesgo su vida. Por lo anterior, se han desarrollado técnicas y equipos quirúrgicos avanzados que permiten identificar estructuras anatómicas clave y reducir el riesgo de complicaciones, como el microscopio robótico o exoscopio. CASO CLÍNICO: Paciente masculino de 48 años con bocio multinodular gigante en quien se practicó una hemitiroidectomía derecha asistida por el microscopio robótico RoboticScope para facilitar la identificación de las estructuras anatómicas clave. La intervención quirúrgica se efectuó en el Hospital General Dr. Manuel Gea González en junio de 2024. El microscopio robótico facilitó la identificación precisa del nervio laríngeo recurrente, la glándula paratiroidea inferior derecha y la arteria tiroidea inferior, lo que permitió una disección precisa y segura. Los resultados y la calidad del uso del exoscopio se valoraron con la herramienta de evaluación de calidad del exoscopio. CONCLUSIONES: El uso del microscopio robótico es prometedor para la cirugía de cuello porque mejora la identificación de estructuras anatómicas clave, reduce complicaciones y proporciona ventajas ergonómicas. Aunque tiene limitaciones como la curva de aprendizaje y el costo, sus beneficios superan las desventajas.
