Abstract (English): Background: Differentiating among the various neoplasms of the urinary and reproductive system in adult patients is essential for establishing an accurate diagnosis and determining the appropriate therapeutic strategy. Clinical case: 71-year-old man with a history of Gleason 10 (5+5) prostate adenocarcinoma who developed a suspicious lesion at the vesicoprostatic junction, initially considered a urothelial carcinoma based on cystoscopy findings and immunohistochemistry. Following a multidisciplinary evaluation, a radical cystoprostatectomy with lymphadenectomy was performed, which subsequently revealed a solitary fibrous tumor of the prostate, a rare entity with benign behavior. Immunohistochemical analysis was key to the final diagnosis, differentiating this benign lesion from the initially suspected urothelial origin. Conclusions: This case underscores the importance of a comprehensive and meticulous approach in the evaluation of atypical prostate lesions, as misdiagnoses can lead to overtreatment. The experience reported here highlights the need to consider solitary fibrous tumors in the differential diagnosis of prostatic stromal tumors. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción: la diferenciación entre las diversas neoplasias del aparato urinario y reproductor en pacientes adultos es fundamental para establecer un diagnóstico preciso y determinar la estrategia terapéutica adecuada. Caso clínico: paciente varón de 71 años con antecedentes de adenocarcinoma prostático Gleason 10 (5+5) que desarrolló una lesión sospechosa a nivel de la encrucijada vésico-prostática, inicialmente considerada como un carcinoma urotelial por hallazgo en cistoscopía e inmunohistoquímica. Tras una evaluación multidisciplinaria, se realizó una cistoprostatectomía radical con linfadenectomía, en la que se identificó posteriormente un tumor fibroso solitario de la próstata, una entidad poco frecuente y con comportamiento benigno y agresivo; sin embargo, también se puede presentar su contraparte maligna, que puede ser muy agresiva en cuanto a su comportamiento metastásico. La revisión inmunohistoquímica fue clave para el diagnóstico final, que diferenció esta lesión benigna del origen urotelial inicialmente sospechado. Conclusiones: este caso subraya la importancia de un abordaje integral y exhaustivo en la evaluación de lesiones atípicas de la encrucijada vésico-prostática, ya que diagnósticos diferenciales erróneos y lo poco frecuente de esta lesión pueden conducir a sobretratamientos y tratamientos tardíos. La experiencia aquí reportada resalta la necesidad de considerar tumores fibrosos solitarios en el diagnóstico diferencial de los tumores del estroma prostático. [ABSTRACT FROM AUTHOR]
