| Abstract (English): | Pain in pediatric patients represents a significant healthcare challenge, where successful treatment prevents the onset of long-term physical and psychosocial consequences. A multimodal approach has been shown to improve pain control by addressing all pain aspects. To determine the effectiveness of Cognitive-Behavioral Therapy (CBT) in pain management, a systematic review and meta-analysis were conducted following the PRISMA guidelines, which included studies published between 2014 and 2024 in four databases (PubMed, EMBASE, SciELO, Cochrane) and gray literature from Google Scholar. Randomized controlled trials (RCTs) comparing CBT with traditional controls (psychoeducation, waiting list, or placebo), focusing on pain intensity reduction, were considered. Nine studies were eligible after evaluating 2,964 references and applying selection criteria. The meta-analysis showed a moderate effect size (SMD = 0.77; 95% CI: 0.44-1.11) with high heterogeneity (I² > 80%). A substantial benefit was observed in juvenile idiopathic arthritis, and moderate effects were observed in headache/migraine and functional abdominal pain, with no significant differences for chronic pain and fibromyalgia. We conclude that, although CBT is promising, the methodological heterogeneity and asymmetry highlighted by the Egger test warrant caution. | Abstract (Spanish): | El dolor en pacientes pediátricos representa un desafío importante en salud, dónde el éxito del tratamiento previene la aparición de secuelas físicas y psicosociales a largo plazo. El abordaje multimodal ha demostrado mejorar el control del dolor al abarcar todas las esferas que lo componen. Con el objetivo de determinar la efectividad de la Terapia Cognitivo-Conductual (TCC) en su manejo, se realizó una revisión sistemática y metaanálisis siguiendo la guía PRISMA, que incluyó estudios publicados entre 2014 y 2024 en cuatro bases de datos (PubMed, EMBASE, SciELO, Cochrane) más literatura gris en Google Scholar. Se contemplaron ensayos controlados aleatorizados (ECA) que compararan TCC contra controles tradicionales (psicoeducación, lista de espera o placebo), enfocándose en la reducción de la intensidad del dolor. Tras evaluar 2964 referencias y aplicar criterios de selección, 9 estudios resultaron elegibles. El metaanálisis mostró un efecto moderado (SMD =0,77; IC 95%: 0,44-1,11) con heterogeneidad alta (I² > 80%). Se observó un beneficio sustancial en artritis idiopática juvenil y efectos moderados en cefalea/migraña y dolor abdominal funcional, sin diferencias significativas para dolor crónico y fibromialgia. Concluimos que, aunque la TCC es prometedora, la heterogeneidad metodológica y la asimetría señalada por la prueba de Egger aconsejan cautela.
