Abstract (English): The article focuses on the 2025 update of the pediatric resuscitation guidelines issued by the American Heart Association (AHA) and the American Academy of Pediatrics (AAP), which mark a transition from sequential algorithms toward resuscitation guided by measurable physiological targets. These guidelines unify the chain of survival for adults and pediatric patients, emphasize the prevention and early recognition of cardiopulmonary arrest (CPA), and promote interventions based on the individual patient’s hemodynamic response. Changes are highlighted in basic resuscitation techniques, advanced airway management, pharmacotherapy, and hemodynamic monitoring, although many recommendations are based on limited evidence or physiological extrapolations. Finally, it is noted that implementation in Latin America faces challenges due to heterogeneity in resources and training, and that the future points toward personalized resuscitation requiring new educational and technological strategies. Abstract (Spanish): El artículo se centra en la actualización 2025 de las guías de reanimación pediátrica emitidas por la American Heart Association (AHA) y la American Academy of Pediatrics (AAP), que marcan una transición desde algoritmos secuenciales hacia una reanimación guiada por objetivos fisiológicos medibles. Estas guías unifican la cadena de supervivencia para adultos y pediátricos, enfatizan la prevención y el reconocimiento precoz del paro cardiorrespiratorio (PCR), y promueven intervenciones basadas en la respuesta hemodinámica individual del paciente. Se destacan cambios en técnicas de reanimación básica, manejo avanzado de la vía aérea, farmacoterapia y monitoreo hemodinámico, aunque muchas recomendaciones se basan en evidencia limitada o extrapolaciones fisiológicas. Finalmente, se señala que la implementación en Latinoamérica enfrenta desafíos por la heterogeneidad en recursos y capacitación, y que el futuro apunta a una reanimación personalizada que requiere nuevas estrategias formativas y tecnológicas.
